摘要
目的通过调查深圳市水痘病例的疾病经济负担,为制定该市水痘免疫策略提供依据。方法在深圳市随机选择两个区,抽样调查所辖区的综合性医院、妇幼保健院和社区健康服务中心的水痘门诊和住院病例,综合评估水痘疾病总经济负担。结果调查2015年5月1日~2016年4月30日期间水痘病例173例,平均医疗费用为(412.5±1082.4)元/例,总经济负担为20.1万元;其中门诊病例161例,平均病程为(8.7±3.7)d/例,平均就诊费用为(157.3±148.7)元/例,总经济负担为12.9万元;住院病例12例,平均病程为(15.8±6.5)d/例,平均就诊费用为(3 835.2±2 065.1)元/例。因住院导致陪护产生的额外费用为(93.6±166.8)元/例,总经济负担为7.2万元。同期深圳市全市水痘门诊病例16 714例、住院病例124例,估算总经济负担为1 411万元。结论深圳市目前水痘经济负担较重,建议通过开展水痘免疫策略的研究加强水痘疫苗的接种,以减轻公众的经济负担。
Objective To investigate the economic burden of varicella,and to provide the evidence for making varicella immunization strategy. Methods Two districts were selected in Shenzhen randomly,comprehensive hospitals,women and children’s hospital and community health centers were selected and all the outpatient and inpatient varicella patients were surveyed,so the comprehensive economic burden were evaluated. Results 173 cases were investigated,the average medical fee was（ 412. 5 ± 1082. 4） yuan per case and the total economic burden was 201 thousand yuan. There were 161 outpatients,the length of hospitalization,the average medical fee and the economic burden were（ 8. 3 ± 3. 7） d,（ 157. 3 ± 148. 7） yuan and 129 thousand yuan. There were 12 inpatients,the length of hospitalization,the average medical fee and the economic burden were（ 15. 8 ±6. 5） d,（ 3 835. 2 ± 2 065. 1） yuan and 72 thousand yuan per case respectively. It was estimated there 16 714 outpatients and 124 inpatients in Shenzhen from May 1,2015 to April 30,2016,the total cost was 14 110 thousand yuan. Conclusion The disease burden in Shenzhen is heavey,it is suggested to strength varicella immunization by studying the immunization strategy,to reduce the economic burden of the public.
