图像三维重建软件在结核性脓胸手术中的应用 被引量:3

摘要 目的回顾性分析并观察采用Mimics图像三维重建软件在局限性结核性脓胸微创手术中的应用效果. 方法回顾性分析我院2015 年1月至 2016 年 12月收治的局限性结核性脓胸手术患者30例,按术前是否行图像三维重建分为2个 组. 观察组15例,采用CT增强扫描,应用Mimics 17.0图像处理软件对图像进行三维重建;对照组 15 例,仅采用CT增强扫描获取图像.上述所获图像在微创手术治疗时作为术者的参考. 比较 2 组切口准确率、切口长度、术后疼痛缓解速度、手术时间(手术操作开始至皮肤缝闭时间)及出血量. 结果观察组切口准确率(100.00%,15/15)高于对照组(73.33%,11/15)(x2=4.615, P=0.032);观察组术后疼痛缓解率(100.00%,15/15)高于对照组(73.33%,11/15)(x2=4.615, P=0.032);观察组切口平均长度为(7.55±0.51) cm,较对照组(8.65±0.92) cm短(t=2.527,P=0.024);观察组手术时间为(178.13±6.93) min,较对照组的(184.93±3.86) min 明显缩短(t=3.130,P=0.007);观察组手术出血量为(573.00±138.00) ml,与对照组比较[(613.00±188.00) ml],差异无统计学意义(t=0.888,P=0.389). 结论 Mimics软件对CT图像进行三维重建定位,可以有效提高局限性结核性脓胸微创手术切口准确性、疼痛缓解速度、缩短切口长度、节省手术时间,值得在临床上推广应用. Objective To retrospectively analyze the application effect of Mimics image reconstruction software in minimally invasive surgery for localized tuberculous empyema. Methods The clinical data of 30 patients with localized tuberculous empyema from January 2015 to December 2016 were summarized.They were divided into 2 groups according to whether to perform three-dimensional reconstruction of images before operation.In the observation group, the 15 cases were scanned by computer tomography (CT).The images were processed by Mimics 17.0 image software and reconstructed by 3D reconstruction.In the control group, the 15 cases received images only by CT enhanced scan.The above images were used as reference for minimally invasive surgery.The incision accuracy, the length of incision, the rate of postoperative pain relief, the time of operation (starting from the operation to the end of the operation) and the amount of bleeding were compared between the 2 groups. Results The incision accuracy of the observation group (100.00%, 15/15) was significantly higher than that of the control group (73.33%, 11/15) (x2=4.615, P=0.032).The rate of postoperative pain relief of the observation group (100.00%, 15/15) was higher than that of the control group (73.33%,11/15) (x2=4.615, P=0.032).The average length of incision in the observation group ((7.55±0.51) cm) was shorter than that of the control group ((8.65±0.92) cm) (t=2.527, P=0.024).The operation time of the observation group ((178.13±6.93) min) was shorter than that of the control group ((184.93±3.86) min) significantly (t=3.130, P=0.007).The amount of bleeding in observation group was (573.00±138.00) ml, compared with the control group ((613.00±188.00) ml), the difference was not statistically significant (t=0.888, P=0.389). Conclusion3D reconstruction of the location of CT image by Mimics software can improve incision accuracy effectively and make pain relief quickly, shorten the length of incision and save the operation time in the minimally invasive surgery for the localized tuberculous empyema, which is worth popularizing in clinic.
