摘要
目的评价武汉市基层医疗卫生机构基本公共卫生服务效率。方法采用数据包络分析(DEA)的C C R模型、B C C模型对2016年武汉市16个区基层医疗卫生机构的2项投入指标[每10万人拥有公共卫生执业医师数(人)、财政下拨经费(万元)]和5个产出指标(高血压健康管理人数、糖尿病健康管理人数、老年人健康管理人数、0~6岁儿童健康管理人数、孕产妇健康管理人数)进行效率分析。结果 2016年武汉市基本公共卫生服务总体效率、纯技术效率和规模效率均值分别为0.873、0.972、0.898。31.25%的地区DEA总体有效,43.75%的地区纯技术效率为1,25%的地区纯技术效率和规模效率均小于1。结论武汉市基层医疗卫生机构基本公共卫生服务总体效率较高,项目管理水平有待提高,卫生资源投入需要增加,基层医疗卫生机构规模需合理扩大。
Objective To evaluate the efficiency of the implementation efficiency of basic public health service in grass-root health institutions in Wuhan. Methods The CCR model, BCC model of DEA was established by using the data of 2016 from primary-level medical institutions in Wuhan. Efficiency analysis were conducted on 2 input indexes, including the number of public health practitioner per 100 000 residents and financial input, and 5 output indexes, including the number of hypertensive patients receiving standard management, the number of diabetic patients receiving standard management, the number of elders receiving standard management, the number of children under 6 receiving standard management, the number of the pregnant women receiving standard management. Results The average values of the comprehensive technological efficiency, pure technical efficiency and scale efficiency of the implementation of basic public health servers in Wuhan were 0.873、0.972、0.898 respectively. The comprehensive technological efficiency of 31.25% district was 1; the pure technological efficiency and scale efficiency of 25% district were all less than 1. Conclusion The comprehensive technological efficiency in basic public health was high in Wuhan, we must improve management level, increase the health resources and expand the scale of primary-level medical.
