摘要
目的:分析不同类型胎儿骶尾部肿块的产前超声声像图特征。方法:应用GE-Voluson E8或GE-Voluson E6彩色多普勒超声诊断仪对20例经MRI或致死性引产证实为胎儿骶尾部肿块的孕妇进行检查,收集其超声声像图结果,分析不同类型肿块的超声声像图特点。结果:20例经证实胎儿骶尾部肿块的孕妇产前超声诊断胎儿开放性脊柱裂脊膜和(或)脊髓膨出9例(45%),闭合性脊柱裂脊膜膨出3例(15%),骶尾部畸胎瘤7例(35%),骶尾部畸胎瘤合并开放性脊柱裂脊髓脊膜膨出1例(5%);开放性脊柱裂伴脊膜和(或)脊髓膨出声像图表现为多切面扫描检查时均可观察到椎骨及皮肤全层不完整,缺损处可见囊性或混合性包块向背侧膨出;闭合性脊柱裂脊膜膨出表现为皮肤完整,脊柱缺损处背部皮下膨出一囊性包块;骶尾部畸胎瘤声像图表现为病变部位皮肤连续,脊柱尾端走形自然,包块内部回声杂乱。结论:不同类型胎儿骶尾部肿块有其独特的超声声像图特征,产前超声可对胎儿骶尾部肿块的类型进行诊断及鉴别。
Objective: To analyze the prenatal ultrasonographie features of different types of fetal sa- crococcygeal mass. Methods: 20 cases of pregnant women with MRI or lethal induction confirmed fetal sacrococcygeal tumors were examined by the application of GE-Voluson E8 or GE-Voluson E6 color Doppler ultrasound diagnostic apparatus. The uhrasonographic results were collected, and the ultra- sonographic characteristics of different types of tumors were analyzed. Results: In 20 cases of confirmed fetal sacrococcygeal mass, prenatal uhrasonography was performed in 9 cases (45 % ) of open fetal spi- nal meninges and(or) spinal cord, 3 cases ( 15% ) of closed spinal bifida meningocele, 7 cases with sacrococcygeal teratoma (35%) and 1 case (5%) of sacrococcygeal teratoma with open spina bifida spinal cord meningocele. The sonographic features of open spina bifida with spinal meninges and (or) spinal cord were characterized by a complete scan of the entire vertebrae and skin, and the defect of visible cystic or mixed enclosed mass dorsally bulged. Closed spina bifida was characterized by a com- plete skin, a cystic mass at the back of the spine and a subcutaneous bulge at the back. Sacrococcy- geal teratoma ultrasonography showed the lesion of skin, spinal end shape nature and mass internal echoes. Conclusion:Different types of fetal sacrococcygeal mass have their unique sonographic fea- tures, and prenatal ultrasound can diagnose and differentiate the types of fetal sacrococcygeal mass.
