长沙市2012-2015年出生缺陷医院监测结果分析 被引量:10

Hospital-based surveillance results of birth defects in Changsha City,2012-2015

摘要 目的了解长沙市2012-2015年出生缺陷医院监测发生情况,为制定长沙市出生缺陷干预措施提供科学依据。方法根据国家卫计委制定的《中国妇幼卫生监测方案》的方法要求,以监测医院为基础收集出生缺陷监测资料,对长沙市2012-2015年13家监测医院的数据进行统计和分析。结果监测围产儿总数192 113例,其中缺陷儿6 878例,出生缺陷发生率为358.02/万。男性出生缺陷发生率(389.05/万)高于女性(321.29/万),差异有统计学意义(χ~2=63.881,P<0.05);城镇出生缺陷发生率(370.23/万)高于乡村(342.62/万),差异有统计学意义(χ~2=10.464,P<0.05);<20岁组和≥35岁组孕母出生缺陷发生率高于其他年龄组。前五位出生缺陷分别为:先天性心脏病、外耳其他畸形、多指(趾)、马蹄内翻足、尿道下裂。结论长沙市的出生缺陷发生率处于较高水平,应当进一步加强出生缺陷的三级预防,减少出生缺陷的发生。 Objective To investigate the incidence of birth defects based on hospital surveillance in Changsha City from 2012 to 2015 so as to provide scientific evidence for formulating birth defect intervention measures in Changsha City. Methods According to the requirement of Maternal and Child Health Surveillance Scheme in China provided by the National Health and Family Planning Commission, we collected the data of birth defects in 13 monitored hospitals in Changsha City from 2012 to 2015, and then statistically analyzed these data. Results A total of 192,113 perinatal infants were monitored. Among them, 6,878 cases of birth defects were detected, and the incidence rate of birth defects was 358.02/10,000. The incidence rate of birth defects was higher in the males than in the females 1389.05/10,000 vs. 321.29/10,000) , and the difference was statistically significant (Х^2= 63.881, P〈0.05). The incidence rate of birth defects was higher in the urban areas than in the rural areas (370.23/10,000 vs. 342.62/10,000 ), with a statistically significant difference (Х^2= 10.464, P〈0.05). The incidence rates of the age groups under 20 years and greater than or equal to 35 years were higher than those of other age groups. The top five common birth defects were congenital heart disease, external ear malformation, polydactyly, clubfoot and hypospadias. Conclusions The incidence rate of birth defects in Changsha City is at a high level; and hence, it is necessary to further strengthen tertiary prevention so as to reduce the incidence of birth defects.
