MR弥散加权成像ADC值在颅内占位性病变诊断中的临床价值 被引量:1

摘要 目的研究弥散加权成像表观扩散系数(ADC值)在颅内占位性病变诊断及鉴别诊断中的应用价值。方法选取在本院2015年1月-2016年6月行MRI检查并术后证实颅内肿瘤患者80例,其中胶质瘤Ⅰ~Ⅱ级4例,Ⅲ~Ⅳ级9例;淋巴瘤12例,其中原发8例,继发4例。脑膜瘤11例(术后病理分型均为良性);神经鞘瘤11列:包括听神经瘤6例,三叉神经瘤4例,面神经瘤1例;脑脓肿24例;颅内结核9例。所有受试者进行MR常规序列及弥散加权成像(DWI)检查,DWI扫描结束后选取感兴趣区,测定病变ADC值,对不同病变ADC值进行统计学分析。结果神经鞘瘤的ADC值最高,其次为胶质瘤结核瘤、脑膜瘤,淋巴瘤,脑脓肿的ADC值最低。在ADC图上,脑膜瘤呈等信号;其次为胶质瘤、结核瘤呈高信号;神经鞘瘤最高,淋巴瘤为低信号,脑脓肿的信号最低,各种颅内占位性病变之间的表观扩散系数(ADC)比较具有统计学意义(P<0.05)。结论常规MRI检查能够对颅内占位性病变形态学特征进行分析,ADC值可对颅内病变的定性诊断、鉴别诊断提供量化的影像学依据。 Objective To study the application values of diffusion weighted imaging apparent diffusion coefficient(ADC) in diagnosis and differential diagnosis of intracranial space-occupying lesions. Methods 80 cases who had MRI and were confirmed intracranial tumor in our hospital between January 2015 to June 2016 were selected. All cases included 4 cases with grade Ⅰ ~ Ⅱ of glioma,9 cases with grade Ⅲ ~ Ⅳ,12 cases with Lymphoma,8 cases with primary glioma,4 cases with secondary tumor,11 cases with meningioma(all postoperative pathologic classification were benign),11 cases with Schwannoma which included 6 cases with acoustic neuroma,4 cases with trigeminal nerve tumor and 1 case with facial nerve tumor,24 cases with brain abscess,9 cases with intracranial tuberculosis. All cases were checked with MR generic sequence and diffusion weighted imaging(DWI). ROIs were selected after DWI scanned,and ADC values of lesions were determined. ADC values of different lesions were analyzed statistically. Results ADC values of nerve sheath tumor were the highest,and followed by glioma tuberculoma,meningioma,lymphoma. ADC values of brain abscess were the lowest. On ADC map,meningioma was in equal signals;followed by glioma,but tuberculoma showed high signals;The signals of schwannoma were highest. lymphoma showed low signals. S signals of brain abscess was the lowest. Apparent diffusion coefficients(ADC)among all sorts of intracranial space-occupying lesions were statistically significant(P 0.05). Conclusion MR generic sequence can analyze the morphological features of intracranial space-occupying lesions, ADC values can provide quantitative imaging basis on qualitative diagnosis and differential diagnosis of the intracranial lesions.
