摘要
【目的】以导管法冠状动脉造影(CAG)为“金标准”,探讨多层螺旋CT冠脉钙化积分与动态心电图监测在冠心痛早期诊断中的价值。【方法】选取2013年4月至2014年1月本院收治的冠心病患者共55例,所有患者均进行CT造影(CTA)和动态心电图检测,将其结果-9CAG比较,分析两种不同诊断方式的敏感度、特异度等。【结果】动态心电图监测敏感度为97.06%(33/34),特异度为90.48%(19/21),阳性预测值为94.29%(33/35)。多层螺旋CT冠脉钙化积分监测敏感度为100.00%(32/32),特异度为86.96%(20/23),阳性预测值为91.43%(32/35)。组间差异无统计学意义(P〉O.05)。【结论】动态心电图与多层螺旋CT冠脉钙化积分对于冠心病的早期诊断均有较高准确性,且心电图操作简便,可重复操作,无创伤,更具推广意义。
[Objective]Catheter coronary angiography was set as the "gold standard" to investigate the value of multi-slice spiral CT coronary calcification score and dynamic electrocardiogram (ECG) monitoring in the early di- agnosis of coronary artery disease. [Methods] A total of 55 cases of patients with coronary heart disease in our hospital from April 2013 to January 2014 were collected for the study. All patients underwent CT angiography (CTA, for multi-slice spiral CT coronary calcification score) and dynamic ECG detection. The results were com- pared with those of CAG "gold standard". The sensitivity and specificity of the two different diagnostic methods were analyzed. [Results]The sensitivity of dynamic ECG monitoring was 97.06% (33/34)% the specificity was 90.48% (19/21), and the positive predictive value was 94.29% (33/35). The sensitivity of multi-slice spiral CT coronary angiography was 100% (32/32)% the specificity was 86.96% (20/23), and the positive predictive value was 91.43% (32/35). There were no statistical differences between the two groups ( P 〈0.05).[Conclusion]Both dynamic electrocardiogram and multi-slice spiral CT coronary calcification score are highly accurate for early diag- nosis of coronary heart disease. Dynamic ECG monitoring, however, is more practical in clinical settings due to its simplicity, repeatability, and non-invasive nature.
