摘要
目的观察飞秒激光小切口透镜取出术(Femtosecond laser small incision lenticule extraction,SMILE)在矫正散光中的疗效。方法 2015年8月至2016年3月,近视散光患者65例130只眼,随机分为两组,实验组患者33例66只眼,散光(-1.26±0.6)D,采用SMILE矫正屈光不正;对照组患者32例64只眼,散光(-1.34±0.7)D,采用波前像差引导的飞秒激光LASIK手术(Wavefront guided femtosecond laser assisted,LASIK)。术后常规处理,术后1和7 d,1、3和6个月常规复查,记录裸眼视力、非接触式眼压、电脑验光、裂隙灯检查、Pentacam(眼前段分析系统)检查。比较两组患者术前和术后6个月的散光矫正疗效。结果两组患者手术顺利,未出现最佳矫正视力下降、角膜瓣移位、角膜感染、严重的偏中心切削等并发症。实验组术前散光(-1.26±0.62)D,术后6个月散光(-0.44±0.3)D,与术前散光进行比较,差异有统计学意义(P<0.05);对照组术前散光(-1.34±0.7)D,术后6个月散光(-0.42±0.3)D,与术前进行比较,差异有统计学意义(P<0.05);实验组和对照组术后6个月散光镜性比较,两组差异无统计学意义(P>0.05)。结论 SMILE安全有效,在矫正散光中能取得与波前像差引导的飞秒激光LASIK手术一致的矫正效果。
Objective To observe the therapeutic effect of femtosecond laser small lenticule extraction( SMILE) on astigmatism.Methods A prospective study was made on 130 eyes( 65 patients) given refractive surgery in the treatment of myopia and astigmatism from Aug. 2015 to Mar. 2016. The patients were randomly divided into two groups: the experimental group( 33 patients,66 eyes),of which the patients had an astigmatism of(-1. 26 ± 0. 6) D and were given correction of myopia and astigmatism with femtosecond laser small incision lens extraction( SMILE),the control group( 32 patients,64 eyes),of which the patients had an astigmatism of(-1. 34 ± 0. 7) D and were given wavefront-guided femtosecond laser LASIK( FS-LASIK). The two groups were given conventional treatment after the surgery. A routine check was performed respectively on the 1st and 7th day and the 1st,3rd and 6th month after the operation and the uncorrected visual acuity,noncontact intraocular pressure,computer optometry,slit lamp examinationand Pentacam( anterior segment analysis system) were measured and recorded. The astigmatism measured with computer optometry was compared between the two groups before and after the operation.Results The operation of both groups was successful and no complication occurred,including but not limited to decrease of best corrected visual acuity,corneal flap displacement,corneal infection and severe central cutting. In the 6th month after the surgery,the astigmatism of the experimental group was(-0. 44 ± 0. 3) D. Compared with the preoperative astigmatism,the difference was statistically significant( P〈0. 05). In the 6th month after the operation,the astigmatism of the control group was(-0. 42 ± 0. 3) D. Compared with the preoperative astigmatism,the difference was statistically significant( P〈0. 05). In the 6th month after the operation,there was no significant difference between the experimental group and the control group( P〉0. 05).Conclusions Femtosecond laser incision lenticulewxtraction( SMILE) is safe and effective and may achieve the same result in correcting astigmatism as wavefront-guided femtosecond LASIK.
