摘要
目的探讨运动训练联合无创正压通气(NIPPV)对慢性阻塞性肺疾病(COPD)合并Ⅱ型呼吸衰竭患者肺康复的疗效。方法稳定期COPD合并Ⅱ型呼吸衰竭患者38例,分为康复组(20例)和对照组(18例)。两组患者均给予基础药物、氧疗及疾病管理等常规处理;康复组加用日间运动训练联合NIPPV。治疗前和治疗6个月评估第1秒用力呼气容积占预计值百分比(FEV_1%预计值)、英国医学委员会呼吸困难量表(mMRC)评分、6-min步行距离(6MWD)及BMI。结果随访6个月。康复组治疗后FEV_1%预计值和6MWD较治疗前增高,mMRC评分较治疗前下降(P<0.05)。康复组治疗后FEV1%预计值和6MWD高于对照组,mMRC评分低于对照组(P<0.05)。治疗后两组BMI比较差异无统计学意义(P>0.05)。结论运动训练联合NIPPV能有效促进COPD合并Ⅱ型呼吸衰竭患者的肺康复,提高重症COPD患者的通气功能和生活质量。
Objective To investigate the efficacy of exercise training combined with noninvasive positive pressure ventilation(NIPPV)in pulmonary rehabilitation of chronic obstructive pulmonary disease(COPD)patients with typeⅡrespiratory failure.Methods A total of 38 stable COPD patients with typeⅡ respiratory failure was divided into two groups of A(20cases)and B(18cases).All patients received basal drug treatment,oxygen therapy and disease management.The patients in group A additionally received exercise training combined with NIPPV.Then the percentage of predicted forced expiratory volume in one second(FEV1%pre),modified medical research council dyspnea scale(mMRC)score,six-minute walking distance(6MWD)and BMI were observed between two groups before and 6 months after treatment.Results The patients were followed up for 6 months.The FEV_1% pre and 6MWD were higher and mMRC score was lower after treatment than those before treatment in group A(P〈0.05).After treatment,the FEV_1% pre and 6MWD were higher and mMRC score was lower in group A than those in group B(P〈0.05).There was no significant difference in BMI between two groups after treatment(P〉0.05).Conclusion Exercise training combined with NIPPV can effectively improve pulmonary rehabilitation,ventilation function and quality of life in COPD patients with typeⅡ respiratory failure.
