摘要
目的探讨L型角钢板(LSCP)、AO动力髁螺钉(DCS)及解剖型髁支持钢板(AC—SP)对老年股骨远端骨折患者的临床疗效。方法选择在我院确诊的96例股骨远端骨折老年患者进行随机分组对照研究,LSCP组35例患者予以L型角钢板固定治疗,DCS组32例患者予以AO动力髁螺钉治疗。ACSP组29例患者予以解剖型髁支持钢板治疗。术后6个月进行随访。通过比较手术治疗后三组患者临床症状以及相关指标来探讨三种内固定方法对老年股骨远端骨折的临床疗效。结果治疗后,ACSP组的并发症情况少于DCS组(P〈0.05);DCS组的并发症情况少于LSCP组(P〈0.01)。治疗后,ACSP组的骨愈合情况优于DCS组(P〈0.05);DCS组的骨愈合情况优于LSCP组(P〈0.01)。治疗后,ACSP组的HSS功能评定优于DCS组(P〈0.01);DCS组的HSS功能评定优于LSCP组(P〈0.01)。结论解剖型髁支持钢板的内固定效果优于AO动力髁螺钉和L型角钢板,值得临床推广应用。
Objective To investigate the clinical effects of L - shaped angle plate (LSCP), AO dynamic condylar screw (DCS) and anatomical condyle plate (ACSP) in the treatment of distal femoral fractures in the elderly. Methods 96 Patients with distal femoral fractures diagnosed in our hospital were randomly divided into LSCP group (n = 35 ), DCS group (n = 32) and ACSP group ( n = 29). LSCP group received LSCP fixation treatment, DCS group accepted DCS therapy and ACSP group received ACSP treatment. The patients were followed up at 6 months after the operation. Clinical symptoms and relating indexes after the operation were compared in the three groups to explore clinical efficacy of internal fixation on distal femoral fractures in the elderly. Results After the treatment, the complications in ACSP group were less than those in DCS group ( P 〈 0. 05 ) ; And the complications in DCS group were less than those in LSCP group (P 〈 0. 01 ). After the treatment, bone healing in ACSP group was better than that in the DCS group ( P 〈 0. 05 ) ; And the bone healing in the DCS group was better than that in the LSCP group ( P 〈 0. 01 ). After the treat- ment, the HSS function in ACSP group was better than that in DCS group (P 〈 0. 01 ) ; And the HSS function in DCS group was better than that in LSCP group (P 〈 0. 01 ). Conclusion The effect of ACSP is superior to DCS and LSCP.
