自然垂降生理盐水柱法经PICC引导腔内心电图的应用研究 被引量:5

摘要 目的探讨对化疗患者采用自然垂降生理盐水柱法经外周静脉穿刺置入中心静脉导管(PICC)引导腔内心电图的应用效果。方法 100例行PICC置管的肿瘤患者根据定位方式不同分为观察组和对照组,各50例。对照组采用超声引导下置管胸部X线摄片定位PICC导管尖端,观察组采用超声引导下置管经自然垂降生理盐水柱引导腔内心电图定位PICC导管尖端。观察不同定位方式对PICC置入最佳位置的影响。结果 100例患者PICC置管后导管尖端均位于上腔静脉中下三分之一处,观察组患者PICC置管尖端达到最佳位置比例高于对照组(P<0.05)。结论自然垂降生理盐水柱法经PICC引导腔内心电图定位操作更加方便,头端定位精准。 Objective To evaluate the application of flowchart of normal saline column by gravity drip via peripherally inserted central catheter( PICC) to guide intracavitary electrocardiograms( ECG). Methods Totally 100 cancer patients undergoing chemotherapy were assigned to the observation group( n = 50) and the control group( n = 50) according to different methods for PICC tip location. Patients in the control group received chest X-ray to guide the tip location after ultrasound-guided PICC catheterization. The flowchart of normal saline column by gravity drip via PICC to guide intracavitary electrocardiograms was adopted in the observation group. Results The PICC tip was located properly( lower 1/3 of superior vena cava) in all 100 patients. The proportion of patients with optimum location of PICC tip was higher in the observation group than that of controls( optimum location). Conclusion Flowchart of normal saline column by gravity drip via PICC to guide intracavitary ECG shows a better efficacy in PICC catheterization,which is advantaged to locating the PICC tip.
