摘要
目的研究异烟肼在结核性胸膜炎患者胸水中的药代动力学。方法 14名符合入选标准的结核性胸膜炎患者随机分为A、B 2组。A组(n=8)给予口服抗结核药物治疗,B组(n=6)在使用A方案同时给予胸腔注射异烟肼0.1g。用LC-MS/MS法测定患者胸水中异烟肼的浓度;用WinNonlin 6.3软件计算主要药代动力学参数。结果两组异烟肼的主要药代动力学参数分别为:C_(max)(1.27±0.52)和(2557.00±965.10)μg·ml-1;tmax(1.79±0.64)和(0.50±0.00)h;t1/2(3.22±1.85)和(5.58±1.42)h;AUC_(0-t)(5.80±3.85)和(1730.38±904.63)h·μg·ml^(-1)。结论口服抗结核药物治疗基础上给予胸腔注射异烟肼,结核性胸膜炎患者胸水中异烟肼的峰浓度显著提高,但t1/2延长并不明显,提示口服抗结核同时局部给药可以提高结核性胸膜炎疗效且不增加药物不良反应。
Objective To study the pharmacokinetics of isoniazid in pleural effusion of patients with tuberculous pleurisy.Methods Fifteen patients of tuberculous pleurisy who met the inclusion criteria were randomly and equally divided into two groups. Group A (n =8) was given antituberculous drugs, while Group B ( n =6) was additionally given pleural injection of isoniazid 0.1 g. Then,the concentration of iso- niazid was determined by LC-MS/MS method. WinNonlin 6.3 software was used to calculate the non-com- partment pharmacokinetic parameters. Results The main pharmacokinetic parameters of Group A and Group B were respectively as follows: C was (1.270.52), (2557.00±965.10) μg .ml 1, t was (1.79±0.64), (0.50±0.00) h, t 1/2 was (3.22±1.85),(5.58±1.42) h,and AUC0-t, was (5.80± 3.85), (1730.38±904.63) h μg .m1-1 .Conclusion Chest injection of isoniazid plus oral antituberculosis drug thera- py can increase, the peak concentration significantly, but the half-life is not prolonged obviously, suggesting that the treatment with pleural injection of isoniazid in patients with tuberculous pleurisy is effective and does not increase side effects.
