摘要
目的分析高血压患者心肌重构与心电图V1导联P波终末电势(PTFV1)的相关性。方法以医院收治的高血压心肌重构患者90例为研究对象,按心电图PTFV1值分为观察组(PTFV1≤-40 mm/s)和对照组(PTFV1>-40 mm/s),超声心动图检测两组心功能,对比两组血清超敏C反应蛋白(hs-CRP)、氮端脑钠肽(NT-proBNP)水平,并分析患者PTFV1绝对值与上述心功能和血清学指标相关性。结果观察组左房内径(LAD)、左室舒张期直径(LVDD)、左室等容舒张时间(LVRT)大于对照组,左心室射血分数(LVEF)、二尖瓣舒张早期流速峰值/二尖瓣舒张晚期流速峰值(E/A)低于对照组(P<0.05);观察组血清hsCRP、NT-proBNP水平高于对照组(P<0.05);相关分析显示,PTFV1绝对值与LVEF呈负相关,与血清hs-CRP、NT-proBNP呈正相关(P<0.05)。结论高血压心肌重构患者LVEF及血清hs-CRP、NT-proBNP与心电图上PTFV1关系密切,心电图PTFV1联合客观指标可用于评估病情发展趋势,为临床诊疗提供依据。
Objective To analyze the correlation between the myocardial remodeling and P terminalforce v1(PTFV1) of ECG in the patients with hypertension.Methods A total of 90 patients with hypertension to receive treatment in our hospital for myocardial remodeling were selected and divided in to an observation group(PTFV1 ≤-40 mm/s) and a control group(PTFV1 -40 mm/s)according to PTFV1 of ECG.The cardiac function in the two groups was detected by UCG.The levels of serum high sensitivity C-reactive protein(hs-CRP) and N-terminal pro brain natriuretic peptide(NT-proBNP) between the two groups were compared.The correlation between the absolute value of PTFV1 and the above cardiac function and serum indexes was analyzed.Results The left atrial diameter(LAD),left ventricle diastolic diameter(LVDD) and left ventricular isovolumetric relaxation time(LVRT) in the observation group were greater than those in the control group,while the left ventricular ejection fraction(LVEF) and mitral valve diastolic early flow peak/mitral valve diastolic peak velocity(E/A) were lower than those in the control group(P〈0.05); the serum hs-CRP and NT-proBNP levels in the observation group were higher than those in the control group(P〈0.05); correlation analysis showed that the absolute value of PTFV1 was negatively correlated to LVEF and positively correlated to serum hs-CRP and NT-proBNP(P〈0.05).Conclusion The LVEF and serum hs-CRP and NT-proBNP of patients with hypertension for myocardial remodeling are closely correlated to PTFV1 of ECG.Combined with objective indexes,PTFV1 of ECG can be used for assessing the development trend of disease,so as to provide basis for clinical diagnosis and treatment.
