摘要
目的观察运动和药物联合物理因子治疗对脊髓损伤后骨质疏松症疗效的影响。方法收集住院康复治疗的脊髓损伤患者30例,其中男性25例,女性5例,患者平均年龄(43.20±16.33)岁。采用随机数字表法分为对照组15例,实验组15例。对照组采用双下肢被动活动或助力运动等运动治疗,每周连续治疗5 d,休息2 d,共治疗24周,并口服钙尔奇D片600 mg,1次/d;骨化三醇0.25μg,2次/d;阿仑膦酸钠70 mg,1次/周,共治疗24周。实验组在上述治疗的同时给予低频脉冲电磁场治疗,强度4 m T、频率20 Hz、40 min/d,每周连续治疗5 d,休息2 d,共治疗24周。治疗前和治疗24周后,分别测定两组患者腰椎L1~4、股骨颈、粗隆区和Ward's三角区骨密度值(BMD),以及血清骨钙素(BGP)的含量。结果与对照组相比,实验组经联合治疗24周后,腰椎L1~4、股骨颈、粗隆区、Ward's三角区骨密度值和血清骨钙素值均明显升高(P<0.05),差异均有统计学意义。结论运动治疗和药物治疗联合物理因子治疗可有效提高骨密度,改善骨代谢,对治疗和缓解脊髓损伤后骨质疏松症的疗效具有积极影响。
Objective To observe the effects of kinesitherapy and pharmacotherapy combined with physiotherapy on spinal cord injury-induced osteoporosis.Methods 30 resident patients with spinal cord injury were selected as the research subjects,including 25 males and 5 females with an average of age(43.20 ±16.33)years old.In accordance with the random number table,patients were divided into the control group and the treatment group.The control group(15 cases)were treated with kinesitherapy,such as passive ROM or assistant exercises for 24 weeks(5-day treatment and 2-day off)and medicine therapy,including oral caltrate D of 600 mg once,daily for 24 weeks,calcitriol of 0.25 μg twice,daily for 24 weeks and alendronate of 70 mg once a week for 24 weeks.The treatment group were treated with above treatment combined with low frequency transduction pulsed electromagnetic fields of 4 m T,20 Hz,40 mins daily for 24 weeks(5-day treatment,2-day off).Before the treatment and 24 weeks after the treatment,the bone mineral density of lumbar vertebra1~4,femur neck,trochanter area and Ward's triangle region and the bone gla-containing protein were examined respectively.Results Compared with the control group,the bone mineral density of above four areas and the bone gla-containing protein increased significantly(P<0.05)after the combination therapy for 24 weeks.Conclusion Kinesitherapy and pharmacotherapy combined with physiotherapy can effectively improve bone mineral density and ameliorate bone metabolism,which has a positive effect on the efficacy of treatment of spinal cord injury-induced osteoporosis.
