持续颅内压监测在双额叶脑挫裂伤治疗中的价值分析 被引量:4

摘要 目的:探讨临床治疗双额叶脑挫裂伤患者行持续颅内监测的重要价值。方法:选择2016年1月-2017年6月笔者所在医院所收治的46例双额叶脑挫裂伤患者为试验组,自入院后均行持续颅内压监测。另选46例患者为对照组,按照2 h每次进行颅内压测定。对比两组患者不同颅内压水平状态下的各种临床治疗效果。结果:患者颅内压越高,相应的不良预后情况越多。试验组患者不良预后31例(67.39%),显著优于对照组(不良预后36例,78.26%),差异有统计学意义(X^2=3.157,P=0.037)。试验组并发症13例,优于对照组的23例,差异有统计学意义(X^2=8.273,P=0.015)。结论:在双额叶脑挫裂伤患者临床治疗过程中,做好持续颅内压监测,可以有效地预测患者病情发展情况,有助于为患者的预后及临床干预措施制定提供参考。 Objective:To investigate the clinical value of continuous intracranial monitoring in patients with bilateral frontal lobe contusion and laceration.Method:46 patients with bilateral frontal lobe contusion and laceration in our hospital from January 2016 to June 2017 were selected as the research object,since were treated with continuous monitoring of intracranial pressure.In addition,46 patients were selected as control group,and intracranial pressure was measured every 2 h.Comparison of two groups of patients with different levels of intracranial pressure under the condition of the clinical treatment effect.Result:The higher the intracranial pressure was,the more the adverse prognosis situation.In the experimental group,31 patients(67.39%) had poor prognosis,which was significantly better than that of the control group(36 patients with poor prognosis,78.26%),the difference was statistically significant( Х^2=3.157,P=0.037).There were 13 cases of complications in the experimental group,and the results were better than 23 cases in the control group,the difference was statistically significant( Х^2=8.273, P=0.015).Conclusion:In the clinical treatment of patient was bilateral frontal brain contusion and laceration,if you can do the continuous monitoring of intracranial pressure,it can effectively predict disease development,contribute to the development for the prognosis of patients and clinical interventions to provide reference.
