摘要
目的探讨高血压脑出血术后精神障碍的发生率及影响因素logistic回归分析,为治疗高血压脑出血术后精神障碍提供临床依据。方法抽取我院神经外科收治的高血压脑出血患者102例作为研究对象,并采用logistic回归分析方法对患者基本资料、家庭情况、社会关系、既往(病)史、脑出血、治疗及术后情况等方面因素进行logistic回归分析,筛选出导致脑出血术后精神障碍发生的危险因素。结果 59例脑出血患者术后出现精神障碍,脑出血术后精神障碍发生率57.84%(59/102);单因素分析结果:性别、家庭关系、经济状况、出血部位、手术方式、基础疾病、预后及改良Barthel指数等是差异性较为显著的因素;多因素回归分析结果:性别、家庭关系、经济状况、手术方式、预后及改良Barthel指数是发生脑出血术后精神障碍的危险因素。结论高血压脑出血术后发生精神障碍是由多种因素共同作用所导致的,对于精神障碍严重患者应及早进行干预治疗。
Objective To investigate the incidence of mental disorders after hypertensive intracerebral hemorrhage and its influencing factors,logistic regression analysis,to provide clinical basis for the treatment of mental disorders after hypertensive intracerebral hemorrhage. Methods 102 patients as the research object from the department of neurosurgery in our hospital of hypertensive cerebral hemorrhage,and regression analysis method in patients with basic information,family situation,social relations,the previous logistic（disease）analysis of history,cerebral hemorrhage,postoperative treatment and other factors in logistic regression,the risk factors which lead to screening mental disorders in patients with cerebral hemorrhage. Results 59 cases of patients with cerebral hemorrhage after operation had mental disorders,57.84% incidence of postoperative mental disorders of cerebral hemorrhage（59/102）;single factor analysis showed：gender,family relationship,economic status,bleeding site,surgical procedure,basic disease,prognosis and modified Barthel index factors are significant differences;multi factor the results of regression analysis：gender,family relationship,economic status,operation,prognosis and modified Barthel index are the risk factors of cerebral hemorrhage postoperative mental disorders. Conclusion Mental disorders after hypertensive intracerebral hemorrhage are caused by many factors,and early intervention should be carried out for patients with severe mental disorders.
