摘要
目的探讨输尿管镜下钬激光联合套石篮治疗输尿管上段结石的疗效。方法选取2014年5月-2015年5月惠州市惠城区中医医院收治的输尿管上段结石患者84例，随机分为对照组和观察组各42例。对照组采用输尿管镜下钬激光治疗，观察组采用输尿管镜下钬激光联合套石篮治疗，观察两组患者手术碎石及并发症情况。结果观察组手术时间长于对照组，住院时间短于对照组，差异有统计学意义（P〈0．05）。观察组碎石成功率和3周结石排净率均高于对照组，差异有统计学意义（P〈0．05）。两组术后感染及输尿管狭窄率差异无统计学意义（P〉0．05）；观察组输尿管穿孔低于对照组，差异有统计学意义（P〈0．05）。结论采用输尿管镜下钬激光联合套石篮治疗输尿管上段结石，碎石成功率及安全性高，值得临床推广应用。
Objective To evaluate the efficacy of ureteroscopic holmium laser lithotripsy combined with basket in the treatment of upper ureteral calculi. Methods A total of 84 cases of upper ureteral calculi patients were collected in our hospital during the period from May, 2014 to May 2015. All patients were randomly divided into control group （n=42, only ureteroscopic holmium laser treatment） and the observation group （n=42, ureteroscopic holmium laser combined with stone basket treatment）. The differences of lithotrity effects and complication were compared. Results There were significantly differences in operative time and hospital stays between control group and observation group （P〈 0.05）. The lithotrity successful rate and 3-week stone clearance rate in observation group were significantly higher than those in control group （P〈0.05）. There are no significant difference in postoperative infection and ureteral stenosis between two groups （P〉0.05）. The ureteral perforation in observation group is significantly lower than control group （P〈 0.05）. Conclusion The ureteroscopic holmium laser combined with stone basket provides high successful rate and safety in the treatment of upper ureteral calculi.
