摘要
目的观察宫颈癌患者血浆游离DNA含量,并分析其与患者临床病理特征的关系。方法选取2014年12月至2016年8月在我院接受治疗的宫颈癌患者为研究对象,同时选取同期在我院接受筛查的非宫颈癌患者作为对照。其中宫颈癌组患者接受1年的随访,观察两组血浆游离DNA含量,观察不同临床病理特征宫颈癌患者血浆游离DNA含量的差异,分析影响宫颈癌患者血浆游离DNA含量的因素。结果宫颈癌患者血浆游离DNA水平高于对照组,差异有统计学意义(P<0.05);临床分期为Ⅲ期、组织学分级为中、低分化、有浸润、有淋巴结转移和死亡组宫颈癌患者的血浆游离DNA含量明显较高,差异有统计学意义(P<0.05),而不同年龄宫颈癌患者的血浆游离DNA含量,差异无统计学意义(P>0.05);将宫颈癌患者血浆游离DNA含量作为因变量,以患者的临床分期、组织学分级、浸润和淋巴结转移作为自变量,进行多因素回归分析。统计分析可得,临床分期、预后和淋巴结转移是影响宫颈癌患者血浆游离DNA含量的因素,其OR值分别为4.658、5.345和5.042。结论宫颈癌患者血浆游离DNA含量较高,且与患者的临床分期、有无浸润、有无淋巴结转移密切相关,可作为临床监测的重要指标。
Objective To observe the plasma free DNA level of cervical cancer patients and investigate its correlation with their clinicopathological features. Methods Patients with cervical cancer received treatment in our hospital from December2014 to August 2016 were selected.Meanwhile,patients without cervical cancer screened in our hospital at the same period were enrolled as control group.Of them,the cervical cancer patients underwent one year of follow-up.The plasma free DNA levels in the two groups were observed.The differences of the plasma free DNA levels in cervical cancer patients with different clinicopathological characteristics were observed.Influencing factors of the plasma free DNA level in cervical cancer patients were analyzed.Results The plasma free DNA level in the cervix cancer patients was higher than that in the control group（P0. 05）.There were significant higher levels of the plasma free DNA in cervical cancer patients with clinical stageⅢ,moderate or low histological differentiation,invasion,lymph node metastasis and death（P〈0. 05）.No significant difference was detected in the plasma free DNA level in different ages of patients（P〉0. 05）.Multiple regression analysis was carried out and the plasma free DNA level in the cervix cancer patients was seen as independent variable and the clinical stage,histological grade,infiltration and lymph node metastasis were seen as dependent variables.Statistical analysis showed that the clinical stage,prognosis and lymph node metastasis were the influencing factors of the plasma free DNA level in the cervix cancer patients and the OR values were 4. 658,5. 345 and 5. 042 respectively.Conclusion The plasma free DNA level in the cervix cancer patient is higher and it has a close correlation with clinical stage,infiltration and lymph node metastasis and can be regarded as an important index in clinical monitor.
