摘要
目的探讨中西医序贯保守治疗卵巢黄体破裂的疗效及安全性。方法将收治的68例卵巢黄体破裂患者随机分为序贯组和对照组,各34例。对照组单纯西医治疗,序贯组在此基础上结合病情发展阶段给予中医序贯治疗(不稳定期采用固本止崩汤行益气摄血止血治疗,稳定期采用血府逐瘀汤行气化瘀消癥治疗)。比较两组的临床疗效、血红蛋白水平、中转手术率和不良反应。结果序贯组总有效率为94.1%,显著高于对照组的75.0%(P<0.05);治疗后,序贯组血红蛋白水平与治疗前无显著差异(P>0.05),但明显高于对照组(P<0.05);序贯组中转手术率为9.8%,显著低于对照组的32.4%(P<0.05);两组治疗期间均未见明显不良反应。结论中西医序贯治疗卵巢黄体破裂是安全可行的,可稳定内出血,提高保守治疗成功率。
Objective To explore the effect and safety of sequential conservative therapy with traditional Chinese medicine combined with western medicine（ TCM-WM） in treatment of corpus luteum rupture. Methods 68 patients with corpus luteum rupture were divided into sequential group（ n = 34） and control group（ n = 34）. Patients in control group were treated with western medicine,and patients in sequential group were given with TCM sequential therapy according to the developmental stages of illness. The clinical effect,stability of bleeding,rates of conversion to operation and side effects were compared between these two groups. Results The total effect rate of sequential group was 94. 1%,and that of control group was 75. 0%,and the difference between these two groups was significant（ P〈0. 05）. After treatment,the level of Hb in sequential group was not significantly reduced compared with that before treatment,while the level of Hb was significantly higher than that of control group（ P〈0. 05）. The rate of converting to operation in sequential group was 9. 8%,which was significantly lower than that（ 32. 4%）of control group（ P〈0. 05）. There was no obvious side effect during the treatment course in these two groups. Conclusion The application of TCM sequential conservative therapy for treatment of patients with corpus luteum rupture is safe and feasible,it can contribute to stablize the hemorrhage,and improve the successful rate of conservative treatment.
