导管溶栓技术在急性下肢深静脉血栓形成中的应用 被引量:8

摘要 目的探讨导管接触性溶栓技术治疗急性下肢深静脉血栓形成(DVT)的临床效果。方法49例急性下肢DVT患者采用导管接触性溶栓技术,持续泵入尿激酶溶栓(80万U/天),应用健患侧肢体周径差、静脉通畅评分及静脉通畅率指标评估疗效。结果本组49例患者均置管成功,术后无严重并发症。溶栓前后健患侧大腿周径差、小腿周径差及静脉通畅评分比较,差异均具有统计学意义(P<0.01);平均静脉通畅率为(57.29±11.38)%。37例(75.5%)患者获得随访,随访时间3~24个月,瓣膜保存率达86.5%。随访期间没有血栓复发及肺栓塞病例。结论导管溶栓治疗急性下肢DVT并发症少、安全有效,临床效果满意,值得推广应用。 Objective To evaluate the clinical effect of catheter directed thrombolysis (CDT)in the treatment of acute deep venous thrombosis of lower limbs. Methods A total of 49 DVT patients applied CDT. Urokinase was infused to thrombolysis constandy with total of 800 thousand units per day. The circumferences of healthy or affected limbs, the venous patency score and the rate of venous patency improvement were evaluated by venography. Results 49 patients with DVT were successfully catheterized. No severe complication occurred during the thrombolysis. After CDT,it was showed that the venous patency improved significantly (57.29± 11.38%), and the differences of limbs' circumferences were significant (P〈0.01). 37 patients were followed up for 3 to 24 months, and it was shown the valvular function was preserved in 32 patients (86.5%). There was no recurrence of thrombosis and pulmonary embolism occurred during the follow-up. Conclusion CDT is a safe treatment with good curative effect in acute deep venous thrombosis of lower limbs, and it is worthy of clinical promotion and application.
