摘要
目的探讨针对性护理干预在预防脑卒中患后睡眠呼吸暂停低通气综合征(SAHS)中的作用。方法选取2016年2月—2017年1月期间收治的78例脑卒中患者,随机分为观察组和对照组,各39例。对照组给予神经内科常规治疗和护理,观察组在对照组基础上根据临床症状给予针对性护理干预。观察2组SAHS发生情况及治疗前后最长呼吸暂停时间、最低血氧饱和度、情绪和睡眠情况。结果治疗后,2组患者的最低血氧饱和度均高于治疗前,最长呼吸暂停时间均短于治疗前,观察组最低血氧饱和度高于对照组,最长呼吸暂停时间短于对照组,差异均有统计学意义(P<0.05)。观察组呼吸睡眠暂停综合征发生率30.77%(12/39),低于对照组的64.10%(25/39),差异具有统计学意义(P<0.05)。治疗后,2组患者的匹兹堡睡眠质量指数量表(PSQI)、Epw orth嗜睡量表(ESS)、焦虑自评量表(SAS)和抑郁自评量表(SDS)评分均较治疗前降低,且观察组低于对照组,差异均有统计学意义(P<0.05)。结论对脑卒中患者进行针对性护理干预,能改善患者的睡眠质量和情绪状况,预防SAHS的发生。
Objective To investigate the targeted nursing in prevention o f sleep apnea- hypop-nea syndrome (SAHS) in patients witti cerebral apoplexy. Methods T otally 78cerebral apoplexy were assigned to the observation group and the control group,w ith 39 cases ineach group. Both two groups received comprehensive treatment and routine care,and the targeted nursing intervention in prevention o f SAHSwas performed in the observation group. The in c i-dence of SAHS,maximum duration o f apnea,mental status and sleeping quality o f patients were observed and compared between two groups. Results Patients in both two groups haminimal oxyhemoglobin saturation and shorter maximum duration o f apnea after treatment,andpatients in the observation group had a better outcome in minimal oxyhemogmaximum duration of apnea (P〈0.05). The incidence o f SAHS was 30.77X in the observation group and was 64. 10 X in the control group (P 〈0.05). Patients in the obser-vation group a-chieved a lower score in Pittsburgh Sleep Quality Index scale (PSQI ),the Epworth S(ESS),self-rating anxiety scale (SAS) and self-rating depression scale (SDS),w ith significant difference (P〈0. 05). Conclusion Application o f targeted nursing intervention may improvesleeping quality and mental status o f patients after cerebral apoplexy,and reduce the ,and reduceapnea-hypopnea syndrome.
