摘要
目的探究乌司他丁配合高频机械通气治疗急性呼吸窘迫综合征(ARDS)的疗效及对肺功能的改善作用。方法选取本院2015年5月至2017年4月收治的80例ARDS患者为研究对象,按随机数表法将其分为观察组(n=40)和对照组(n=40);对照组采用高频机械通气治疗,观察组在对照组基础上联合乌司他丁治疗;比较两组呼吸机相关性肺炎(VAP)发生情况、死亡情况和治疗前后肺功能及血气分析指标变化。结果观察组VAP发生率和病死率均显著低于对照组(5.00%比22.50%,2.50%比12.50%),差异具有统计学意义(均P〈0.05);排除治疗过程中死亡患者后,两组患者治疗后FEV1/FVC、PaO2、PaCO2、pH、RR水平较治疗前均有显著改善(均P〈0.05),且观察组治疗后FEVI/FVC、PaO2、PaCO2、pH、RR改善程度显著优于对照组,差异具有统计学意义(均P〈0.05)。结论乌司他丁配合高频机械通气治疗可以显著改善ARDS患者肺功能和通气障碍,抑制肺部炎症反应,降低ARDS患者并发症发生风险,提高患者生存率,安全性较高,值得推广应用。
Objective To explore the efficacy of ulinastatin combined with high frequency mechanical ventilation for acute respiratory distress syndrome (ARDS) and its improvement of pulmonary function. Methods 80 patients with ARDS treated at our hospital from May, 2015 to April, 2017 were selected for the study and divided into an observation group (n=40) and a control group (n=40) by random number table method. The control group were treated with high frequency mechanical ventilation and the observation group with high frequency mechanical ventilation and ulinastatin. The occurrence of ventilator-associated pneumonia (VAP) and mortality and pulmonary function and changes of blood gas analysis indexes before and after treatment were compared between the two groups. Results The incidence of VAP and mortality were 5.00% and 2.50% in the observation group and were 22.50% and 12.50% in the control group, with statistical differences (both P 〈 0.05). After excluding the dead patients during the treatment, the levels of FEV1/FVC, PaO2, PaCO2, pH, and RR were significantly better after than before the treatment in both groups and were better in the observation group than in the control group after the treatment, with statistical differences (all P 〈 0.05). Conclusion Ulinastatin combined with high frequency mechanical ventilation in the treatment of patients with ARDS can significantly improve pulmonary function and respiratory disorder, inhibit lung inflammation, reduce the risk of complications, and improve the patients' survival rate and is safe. Therefore, it is worth being generalized.
