老年女性骨密度与UCOC及双足静力性平衡的关系

摘要 目的探讨老年女性骨密度与血清羧化不全骨钙素(UCOC)浓度以及双足静力性平衡之间的关系。方法选取2016年9—12月在青岛大学附属医院就诊的102例老年女性病人,采用超声骨密度仪测定其跟骨骨密度,依据骨密度分为对照组、骨量减少组和骨质疏松组。采用酶联免疫吸附试验(ELISA)方法测定各组血清UCOC浓度,分析骨密度与血清UCOC浓度的相关性。应用Footscan Balance 7.7平板式足底压力测试系统检测各组双足静止站立时压力中心在X方向及Y方向上的移动距离(Cop X、Cop Y)、压力中心移动轨迹总长度(TTW)以及包绕压力中心移动轨迹的椭圆面积(EA),评价其静力性平衡能力并进行比较。结果骨质疏松组、骨量减少组血清UCOC浓度均较对照组升高,差异具有统计学意义(F=36.24,P<0.05)。3组Cop X、Cop Y比较差异均无显著性,骨质疏松组、骨量减少组TTW、EA较对照组下降,差异有统计学意义(F=30.06、33.22,P<0.05)。结论老年女性骨密度与血清UCOC浓度有密切关系,高水平UCOC可能是骨密度下降的危险因素之一,且老年女性骨密度可能与其静力性平衡能力存在负相关关系。 Objective To investigate the correlation of bone mineral density with the serum level of undercarboxylated osteocalcin(UCOC)and the static balance of the feet in elderly women. Methods A total of 102 elderly women who visited The Affiliated Hospital of Qingdao University from September to December,2016 were enrolled.An ultrasound bone densitometer was used to measure the bone mineral density of the calcaneus,and according to the bone mineral density,they were divided into control group,osteopenia group,and osteoporosis group.ELISA was used to measure the serum level of UCOC,and the correlation between bone mineral density and serum UCOC level was analyzed.Footscan Balance 7.7 flat plantar pressure measurement system was used to measure the displacement of pressure center along the X and Y directions(Cop X and Cop Y)during static standing,total traveled way(TTW)of the pressure center,and ellipse area(EA)which enclosed the TTW of pressure center. Results Compared with the control group,the osteopenia group and the osteoporosis group had a significant increase in the serum level of UCOC(F=36.24,P0.05).There were no significant differences in Cop X and Cop Y between the three groups,and the osteopenia group and the osteoporosis group had significant reductions in TTW and EA compared with the control group(F=30.06 and 33.22,P0.05). Conclusion Bone mineral density is closely correlated with serum UCOC level in elderly women and a high serum UCOC level might be a risk factor for the reduction in bone mineral density.Bone mineral density may be negatively correlated with static balance ability in elderly women.
