摘要
目的分析研究甲状腺自身抗体阳性对试管婴儿(IVF)孕妇妊娠结局的影响。方法选取2013年2月-2015年5月在我院生殖遗传科进行IVF助孕,术前常规检查发现甲状腺功能正常,但甲状腺自身抗体呈阳性,并成功妊娠的30例孕妇作为研究对象,作为观察组,另选取同期IVF术前检查甲状腺功能正常30名,甲状腺自身抗体呈阴性试管婴儿孕妇作为对照组(所有病例均为单胎妊娠)。对比观察两组研究对象妊高症、流产、早产、产后出血、新生儿吸入综合征等不良妊娠结局发生情况。结果两组孕妇妊高症及死胎发生情况比较,差异无统计学意义(P〉0.05);观察组孕妇流产、早产、产后出血、新生儿吸入综合征等不良妊娠结局发生率均高于对照组,差异有统计学意义(P〈0.05)。结论 IVF助孕前甲状腺自身抗体阳性的孕妇较阴性者较易发生流产、早产等不良妊娠结局,因此,对IVF前出现甲状腺自身抗体阳性的试管婴儿孕妇应及早进行干预措施,进而降低不良妊娠结局发生率。
Objective To study the effect of thyroid autoantibodies on the pregnancy outcome of test tube baby.Methods From February 2013-May 2015 in our hospital Reproductive Genetics Section of IVF IVF,routine preoperative examination revealed normal thyroid function,were selected as the observation group.And 30 cases IVF with simultaneous examination of normal thyroid function,thyroid autoantibodies negative IVF pregnant women were selected as a control group (all cases were singleton pregnancies).The incidence of adverse pregnancy outcomes such as pregnancy induced hypertension,abortion,premature birth,postpartum hemorrhage,neonatal aspiration syndrome and other adverse pregnancy outcomes were compared between the two groups.Results Two groups of pregnant women,pregnancy induced hypertension and stillbirth cases showed no signifcant diference (P〉0.05);The incidence of abortion,premature delivery,postpartum hemorrhage,neonatal aspiration syndrome and other adverse pregnancy outcomes in the observation group were signifcantly higher than those in the control group,the diference was signifcant (P〈0.05).Conclusion The pregnant women with positive thyroid autoantibodies in pregnant women are more prone to abortion,premature birth and other adverse pregnancy outcomes than the negative ones.Therefore,the pregnant women with thyroid autoantibodies positive test tube baby should be early intervention measures,and then reduce the incidence of adverse pregnancy outcome.
