摘要 【目的】分析妊娠期间应用抗生素对于新生儿肠道菌群建立的影响。【方法】选取符合入选条件的60例新生儿,将其中孕期存在抗生素暴露的新生儿归为暴露组,并按应用时间分为暴露3 d亚组和暴露7 d亚组各15例,另30例在孕期未接受任何抗生素暴露的新生儿归为非暴露组,在新生儿出生的第1天、第5天通过粪便培养观察新生儿肠道菌群情况。【结果】。暴露组中孕期抗生素使用率最高类别为头孢菌素,占比66.67%,高于其他类型抗生素(P<0.05);暴露组总体G+球菌计数显著高于非暴露组,暴露组G-杆菌显著低于非暴露组,暴露7 d亚组的G-杆菌显著低于暴露3d亚组,组间差异显著(P<0.05);相同时间点,暴露组新生儿粪便大肠埃希菌显著低于非暴露组(P<0.05),。【结论】孕期孕期应用抗生素能够对新生儿肠道菌群正常建立产生破坏性作用,容易引发新生儿肠道微生态紊乱,导致肠道菌群失衡,而抗生素应用时间越长,破坏性效应越显著。 [Objective] To analyze the effect of antibiotic use during pregnancy on the establishment of neonatal intestinal flora. [ Methods ] A total of 60 neonates who fitted the conditions were selected. The neonates with pregnant antibiotic exposure were selected as exposure group. According to the application time, they were divided into 3d exposure subgroup and 7d exposure subgroup, 15 cases per group. The other 30 cases without any pregnant antibiotic exposure were selected as non-exposure group. On the first and fifth days of the birth, neonatal intestinal microflora was observed by fecal culture. [ Results ] The utilization rate of cephalosporin was the highest (66.67%) in pregnant exposure group, and it was significantly higher than those of other antibiotics (P〈0.05). The total G+ cocci counts of the exposure group were significantly higher than those of non-exposure group. The G bacilli counts in pregnant exposure group were significantly lower than those of non-exposure group. There were significant difference between the groups (P〈0.05). At the same time point, in the exposure group, the counts of neonatal fecal Escherichia coli were significantly lower than those of non-exposure group (P〈 0.05), and the statistical difference was significant. [Conclusion] The application of antibiotics during pregnancy can devastatingly affect the establishment of neonatal normal intestinal flora and easily lead to neonatal intestinal microflora disorders. It can result in the intestinal flora imbalance. If the application time of antibiotics is longer, the destructive effect is more obvious.
