摘要
目的:观察参麦注射液在减少血液透析中低血压发生率方面的效果。方法:入选维持性血液透析患者129例(每周至少发生1次透析中低血压)。随机分入参麦组(每次透析期间缓慢滴注参麦注射液100 m L)、米多君组(每次透析前予以米多君2.5~5 mg)和对照组(每次透析期间患者滴注10%葡萄糖或生理盐水100 m L)。观察3周,比较各组低血压的发生率及不良反应。结果:治疗前,3组每周发生低血压的次数分别为:(2.13±1.41)次、(2.26±1.07)次和(2.31±0.68)次。治疗第1周,参麦组和米多君组发生低血压的的次数分别为(0.89±0.44)次和(1.09±0.57)次,与治疗前和对照组(2.45±1.17次)比较,差异均有统计学意义(P<0.01)。治疗第3周,参麦组发生低血压(0.79±0.63)次,与米多君组[(2.07±1.88)次]和对照组[(2.26±1.07)次]比较,差异均有统计学意义(P<0.01)。米多君组出现恶心共28例次,呕吐5例次,头晕16例次,研究结束后抗拒再次使用米多君者11例。参麦组和对照组未见不良反应。结论:参麦注射液可有效减少血液透析中低血压的发生。
Objective： To investigate the effect of Shenmai injection in reducing the incidence of hemodialysis hypotension.Methods： The 129 cases of maintenance hemodialysis patients who experienced dialysis hypotension once a week at least were selected and randomly divided into the SMI group（which were dripped Shenmai injection 100 m L slowly during each dialysis）,midodrine group（which were given midodrine 2.5 mg^5 mg each pre-dialysis） and control group（which were dripped 10% glucose or saline 100 ml during each dialysis）.Observe for three weeks,compare the incidence and adverse reactions of hypotension of each group.Results： Before treatment,the frequency of hypotension weekly of the three groups were：（2.13±1.41） times,（2.26±1.07） times and（2.31±0.68） times.The first week of treatment,the frequency of hypotension of the SMI group and midodrine group were（0.89±0.44） times and（1.09±0.57） times.Compared with treating before and the control group（2.45±1.17） times,it was significantly different（P〈0.01）.The third week of treatment,the frequency of hypotension of the SMI group was（0.79 ±0.63） times.Compared with the midodrine group（2.07 ±1.88） times and the control group（2.26±1.07） times,it was a significant difference（P〈0.01）.Midodrine group had a total of 28 cases of nausea,5 cases of vomiting,16 cases of dizziness,and 11 cases of resisting re-use midodrine after the end of the study.SMI group and the control group,no adverse reactions.Conclusion： Shenmai injection can effectively reduce the incidence of hypotension in hemodialysis.
