奶牛肺外结核病的综合诊断与病原分离鉴定 被引量:4

Integrated Immunological Diagnosis of Extrapulmonary Tuberculosis for Dairy Cows Combining with Pathogenic Isolation and Identification

摘要 为揭示牛型提纯结核菌素(PPD)检测阳性牛与剖检肺脏病变不吻合的真实原因,综合运用皮内变态反应(SIT)、比较变态反应(SICCT)、IFN-γ试验,以及牛型PPD点眼反应、ELISA抗体检测等5种免疫学方法,对我国西部5个奶牛场进行牛结核病综合诊断,然后对锁定的后期感染牛进行扑杀、剖检,采集病料进行病理学和病原学诊断。结果显示:用5种方法综合检测确诊的95头后期感染牛中,有84头(88.42%)剖检可见明显结核病变,其中12头(14.29%)的病变发生在肺和肺门淋巴结,72头(85.71%)的病变发生在肝、脾、肠系膜淋巴结等肺外组织器官;95头后期感染牛中,有54头病料培养阳性;阳性菌经Xpert MTB/RIF和Enigma鉴定,均为牛分枝杆菌。该研究结果表明:我国牛结核病存在肺结核和肺外结核两种,因而增加了牛结核病的诊治难度;牛结核病确诊仅靠单一检测方法十分困难,需运用多种免疫学方法进行综合诊断;该结果可以解释目前出现的检测阳性与肺部剖检病变结果不一致的困惑。 In order to reveal the true reason why there was inconsistence between PPD positive cattle and their autopsy lesions in lungs,5 kinds of bovine tuberculosis immunological diagnosis methods,including SIT,SICCT,IFN-γ test,bovine PPD eye-drops reaction and ELISA antibody detection,were adopted to carry out a synthetic detection towards 5 dairy farms of western China. Then the infected cows at late stage were locked,killed and necropsied,the pathological samples were collected for pathological and etiological diagnosis. Based on the results,among the 95 positive cows that were locked through ?ve detection methods,84 cows(88.42%)were found to have obvious tuberculosis lesions,lesions of 12 cows(14.29%)were in lung and hilar lymph nodes,lesions of the rest 72 cows(85.71%)were found in extrapulmonary tissues and organs,such as liver,spleen and mesenteric lymph nodes,etc. Pathological samples of 54 cows were cultured positive among the 95 cows,and all the positive bacteria were identi?ed as Mycobacterium bovis by Xpert MTB/RIF and Enigma. The results suggested that it was more dif?cult to diagnose and prevent bovine tuberculosis because pulmonary tuberculosis and extrapulmonary tuberculosis were both present in China. It was very dif?cult to use single detection method for diagnosis of bovine tuberculosis,so using integrated immunological diagnosis were essential. And the results also could explain the confusion that the positive detection results were inconsistent with their lung lesions at necropy.
