2型糖尿病及其前期患者胰岛素抵抗与血脂代谢紊乱关系研究 被引量:9

Relationship between Insulin Resistance and Dyslipidemia in Patients with Type 2 Diabetes Mellitus and Its Early Stage

摘要 目的探讨2型糖尿病(T2DM)及其前期患者胰岛素抵抗与血脂代谢紊乱之间的相关性。方法对比分析43例T2DM患者,48例糖尿病前期患者(包括21例空腹血糖受损患者,27例糖耐量减低患者)的口服葡萄糖耐量试验(OGTT)、胰岛素释放试验(IRT)、血脂的测定结果。结果 T2DM组的空腹血糖(FBG)、餐后2h血糖(2h PG)、甘油三脂(TG)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、胰岛素抵抗指数1(HOMA1-IR)显著高于糖尿病前期组(P<0.05),高密度脂蛋白胆固醇(HDL-C)显著低于糖尿病前期组(P<0.05)。Pearson相关分析显示HOMA1-IR与FBG、2h PG、TG、TC、LDL-C呈正相关(P<0.05),与HDL-C呈负相关(P<0.05)。结论 T2DM及其前期患者胰岛素抵抗与血脂代谢紊乱的关系密切。 Objective To investigate the correlation between insulin resistance and dyslipidemia in patients with type 2 diabetes mellitus(T2DM) and prediabetes.Methods The results of oral glucose tolerance test(OGTT), insulin release test(IRT) and blood lipids in 43 cases of T2DM and 48 cases of prediabetes(including 21 cases of impaired fasting glucose and 27 cases of impaired glucose tolerance) were analyzed retrospectively.Results(1)The fasting blood glucose(FBG), 2 h postprandial blood glucose(2 h PG),Triglyceride(TG),Total Cholesterol(TC),Low-Density Lipoprotein Cholesterol(LDL-C) and Insulin Resistance Index(HOMA1-IR) of the T2DM group were significantly higher than those of the prediabetes group(P<0.05),and the High-Density Lipoprotein Cholesterol(HDL-C) were significantly lower than those of the prediabetes group(P<0.05).(2)The Pearson correlation analysis showed that the HOMA1-IR was positively correlated with FBG,2 h PG and TG,TC and LDL-C(P <0.05), and negatively correlated with HDL-C(P <0.05).Conclusion There is close correlation between insulin resistance and dyslipidemia in patients with T2DM and prediabetes.
