摘要
目的探讨MRI动态增强扫描和弥散加权成像在乳腺癌中的诊断价值。方法回顾2015年9月~2017年3月期间我院诊治疑似为乳腺癌的79例患者作为观察对象进行分析,均行DCE-MRI及DWI扫描检查。以手术病理结果为金标准,分析及总结表观弥散系数和时间信号强度曲线对乳腺癌诊断价值。结果 DWI序列b=800s/mm2时,乳腺癌的ADC平均值明显低于良性病变的ADC平均值,其差异有统计学意义(t=1.30,P<0.05);时间-信号强度曲线分型对诊断乳腺癌与良性病变的差异有统计学意义(2=49.78,P<0.05)。结果显示,MRI相关参数对诊断乳腺癌的效能排序:ADC>TIC>MRI形态学。结论 MRI动态增强扫描和扩散加权成像在乳腺肿块良恶性鉴别诊断中具有重要价值,两者联合诊断能明显提高诊断准确性。
Objective To study the diagnostic value of MRI and DWI in breast cancer. Methods We reviewed 79 patients with breast lesions from September 2015 and March 2017, who were performed MRI examination at 3.0 T scanner. 79 lesions were confirmed by histopathology, we analyzed the numbers and features of the lesion with the alidities of ADC value and time-sig- nal intensity curve. Results The average ADC value of malignant tumors was lower than that of benign tmnors. There was statis- tical significant difference of ADC value between malignant tumors and benign tumors ( t = 1.30, P 〈 0.05 ). Studies had shown that MRI-related parameters for the diagnosis of breast cancer performance sort were ADC 〉 TIC 〉 MRI morphology. Conclusion MRI dynamic contrast-enhanced and diffusion-weighted imaging in breast cancer benign and malignant differential diagnosis is of great value. The two combined diagnosis can significantly improve the diagnostic accuracy.
