颈动脉粥样硬化斑块中牙龈卟啉单胞菌菌毛fimA基因型的分布研究 被引量:8

The distribution of fim A genotype of Porphyromonas gingivalis in carotid atherosclerotic lesions

摘要 目的检测牙龈卟啉单胞菌菌毛不同fimA基因型在颈动脉粥样硬化斑块中的分布。方法选取需要进行颈动脉内膜剥脱术的患者47例,收集术中分离的颈动脉粥样硬化斑块和患者龈下菌斑,用16S rRNA PCR检测牙龈卟啉单胞菌菌毛不同fimA基因型的分布。结果在47例患者中,牙龈卟啉单胞菌在龈下菌斑和颈动脉粥样硬化斑块中的检出率分别为72.3%和46.8%,在同一患者同时检出率为40.4%。Type Ⅱ fimA在龈下菌斑和颈动脉粥样硬化斑块中的检出率最高,分别为76.4%和81.8%;其次为Type Ⅳ fimA,检出率分别为50%和40.9%;这两种基因型的检出率显著高于其他基因型。牙龈卟啉单胞菌Type Ⅴ fimA基因型在龈下菌斑和动脉粥样硬化斑块中均未检出。结论牙龈卟啉单胞菌及其不同fimA基因型同时在龈下菌斑和颈动脉粥样硬化斑块中被检出,Type Ⅱ和Ⅳ fimA基因型牙龈卟啉单胞菌作为主要的定植菌,可能在颈动脉粥样硬化发生和发展中起到了一定的作用。 Objective To detect the prevalence of Porphyromonas gingivalis(P.gingivalis) and its fimA genotypes in carotid atherosclerotic lesions. Methods 47 patients scheduled for carotid endarterectomy were enrolled in this study. The atherosclerotic plaque and subjingival plaques were collected. and 16S rRNA PCR was used to detect the different fimA genotypes of P.gingivalis distributed. Results In 47 patients, the detection rates of P.gingivalis in subgingival plaque and carotid atherosclerotic plaque were 72.3% and 46.8%, respectively, and the detection rate was 40.4% in the same patient. Type Ⅱ fimA was most frequently detected in the subgingival plaque(76.4%) and carotid atherosclerotic plaque(81.8%), followed by Type Ⅳ fimA, the detection rates were 50% and 40.9% respectively; Type Ⅴ fimA is detected in both. Conclusion P.gingivalis with various fimA genotypes were present in subgingival plaque and carotid atherosclerotic plaque. Type Ⅱ and Ⅳ fimA of P.gingivalis as the main colonization, may play a roles in the initiation and progression of atherosclerotic plaque.
