摘要
目的评价慢性阻塞性肺疾病并肺动脉高压患者行医院家庭序贯无创通气治疗法的效果。方法选取进行医院无创通气并带有家庭型Bi PAP呼吸机、在家训练或每日坚持在医院进行无创通气的慢阻肺并肺动脉高压患者25例作为研究组;同时选取在治疗期间进行无创通气治疗,暂停治疗后由于各种因素而未进行无创通气的慢阻肺并肺动脉高压患者25例,作为对照组。两组患者进行无创通气或者氧疗的时间在10 h,以6个月为观察周期。50例患者在住院期间均行CAT评分,计算氧合指数,同时进行心脏彩超检查测定,记录患者的肺动脉压指数以及右心室内径,同时治疗6个月后再次行CAT评分等上述指标测定,对比治疗前后的差异性。结果研究组治疗前后的评分分别为(24.1±7.1)分和(20.0±3.9)分;研究组肺动脉压力以及右心室内径比较中,治疗前分别为(59.4±11.5)mm Hg、(28.1±5.5)mm,治疗后分别为(53.1±8.6)mm Hg、(24.5±4.2)mm Hg;对照组治疗前后的肺动脉压对比无统计学意义(P>0.05)。结论采用医院家庭序贯无创通气法对慢阻肺合并肺动脉高压患者的治疗效果显著,值得临床推广应用。
Objective To evaluate the effect of hospital family sequential noninvasive ventilation therapy in patients with chronic obstructive pulmonary disease complicated with pulmonary hypertension. Methods 52 patients with chronic obstructive pulmonary disease complicated with pulmonary disease who were given hospital noninvasive ventilation and carried with family BiPAP ventilator and were given family training or daily noninvasive ventilation in our hospital were enrolled in the study group; at the same time, 25 patients with chronic obstructive pulmonary disease complicated with pulmonary disease who were given noniuvasive ventilation during the treatment period and were not given nonin- vasive ventilation after treatment suspension due to various factors were enrolled in the control group; two groups of patients were given noninvasive ventilation or oxygen therapy, with the time of 10 hours, and 6 months were taken as an observation cycle. 50 patients were all given CAT scores during hospitalization. The oxygenation index was calculated, and at the same time, examination of heart color uhrasonography was performed. The pulmonary artery pressure index and the right ventricular diameter were recorded. At the same time, after 6 months of treatment, CAT score and other above indices were measured again, The differences before and after treatment were compared. Results The scores in the study group were (24.1±7.1) and (20.0±3.9) respectively before and after treatment; in the study group, the pulmonary artery pressure and the right ventricular diameter were compared. The data before treatment were(59.4±11.5) mmHg,(28.1±5.5)mm, and the data after treatment were (53.1±8.6)mmHg and (24.5±4.2)mm respectively; there was no statistically significant difference in pulmonary artery pressure between the two groups before and after treatment(P〉0.05). Conclusion The application of hospital family sequential noninvasive ventilation method in the treatment of patients with chronic obstructive pulmonary disease complicated with pulmonary hypertension has a significant therapeutic effect, it's worth of clinical application.
