福州市地区妇女高危型HPV感染亚型分布情况 被引量:1

Genotyping distribution of high risk human papillomavirus infection in married woman of fuzhou city

摘要 目的:探讨福州市地区妇女高危型人乳头瘤病毒(HR-HPV)感染的亚型分布情况,分析本地区HPV流行亚型,指导适合本地区的宫颈癌疫苗研发.方法:选取2015年12月至2017年1月我院妇产科门诊就诊1073例妇女,以二代杂交捕获法筛查14种HR-HPV亚型的感染情况,阳性样本进一步通过基因芯片检测技术进行亚型鉴定.结果:1073例宫颈分泌物样本共检出HR-HPV阳性161例,阳性检出率为15.00%(161/1073),其中单一型感染103例,检出率9.60%;多重感染58例,检出率5.40%.161例HR-HPV阳性样本共检出227个HPV亚型感染,各亚型分布情况为HPV 52型46例(20%)、HPV 16型28例(12%)、HPV 51型22例(10%)、HPV 58型20例(9%)、HPV33型17例(8%)、HPV 18型17例(7%)、HPV 56型16例(7%)、HPV 66型15例(7%)、HPV 68型12例(5%)、HPV39型12例(5%)、HPV 31型7例(3%)、HPV35型5例(2%)、HPV 45型5例(2%)和HPV 59型5例(2%).各年龄段感染情况:〈30岁年龄段为13.78%,3040岁年龄段为14.47%,4050岁年龄段为16.19%,50~60岁年龄段为15.15%,60岁≥年龄段为23.08%,各年龄段间无统计学差异(χ2=1.657,P=0.798).结论:州市地区目前的感染亚型以HPV52、51、16、58、18和33型居多,HPV52亚型最常见,因此福州地区宫颈癌疫苗应针对流行亚型开发研制适合本地区人群的疫苗. Objective: To investigate the distribution of the genotyping of high risk human papillomavirus (HR-HPV) infection in women of Fuzhou region, analyze the common subtypes, and give guidance for cervical cancer vaccine research and development. Methods: A total of 1073 cases of women attending the outpatient of Department of Obstetrics and Gynecology from December 2014 to January 2016 were enrolled in the study. 14 HR-HPV genotypes were detected by HC2 and then the positive samples were genotyped by Gene Chips. Results: A total of 161 cases were HR-HPV positive in 1073 cases of samples, with positive rate 15.00%(161/1073) . The single infected cases were 103, with positive rate 9.60%, and mixed infected were 58, with positive rate 5.40%. 227 HPV infected subtypes were detected from these 161 HR-HPV positive samples, including 46 cases of HPV52 (20%) , 28 cases of HPV 16 (12%), 22 cases of HPV 51 (10%) , 20 cases of HPV58 (9%) , 17 cases of HPV 33 (8%) , 17 cases of HPV 18 (7%) , 16 cases of HPV 56 (70/6) , 15 cases of HPV 66 (7%) , 12 cases of HPV 68 (5%) , 12 cases of HPV 39 (5%) , 7 cases of HPV 31 (3%) , 5 cases of HPV 35 (2%) , 5 cases of HPV 45 (2%) and 5 cases of HPV 59 (2%) . Age distribution: age〈30 was 13.78%, age 30-40 was 14.47%, age 40-50 was 16.19%, age 50- 60 was 15.15% and age〉60 was 23.08%. There were no significant difference between all age groups (X^2=1.657, P=0.798) . Conclusion: The infection ofHPV genotypes 52, 51, 16, 58, 18 and 33 were more common than another eight subtypes, especially for HPV 52 in Fuzhou. It should design cervical cancer vaccine according to those detected common subtypes.
