摘要
目的分析2015—2016年重庆市恶性肿瘤医保特病办理资料。方法收集2015年1月至2016年12月重庆市恶性肿瘤特病办理数据,对数据整理归类,并作统计分析。结果 60~70岁是重庆市恶性肿瘤特病办理的最高峰,但女性在40岁就开始出现高峰。男性恶性肿瘤特病前三位的是肺癌、结直肠癌、肝癌;女性则是乳腺癌、子宫癌、肺癌。常见恶性肿瘤中甲状腺癌是特病办理中位年龄最小的为44岁;食管癌最大,为65岁。特病资料显示,甲状腺癌是除乳腺癌、子宫癌外唯一女性多于男性的常见恶性肿瘤,其他常见恶性肿瘤则是男性更多。与2012年全国新发恶性肿瘤数据比较,重庆市除2015年子宫癌的构成比与全国数据差异无统计学意义(P>0.05),其余恶性肿瘤的构成比明显区别于全国数据(P<0.05)。而2015与2016年重庆市数据自身比较,除食管癌、甲状腺癌外(P<0.05),相同肿瘤的数量构成比差异无统计学意义(P>0.05)。结论重庆市恶性肿瘤医保特病资料有自身特点,根据常见恶性肿瘤特病办理的性别与年龄特征,应在不同年龄对男女进行差异化肿瘤筛查。
Objective To analyze the data of special medical insurance management （SMIM） for malignant tumor in Chongqing in 2015--2016. Methods Data of SMIM for malignant tumor in Chongqing in 2015--2016 was collected, classifed and analyzed. Results For both men and women, the 60-70 years old is the most common age group in SMIM for malignant tumor. The most common three malignant tumors for men were lung cancer, colorectal cancer and liver cancer, while for women were breast cancer,uterine cancer and lung cancer. Thyroid cancer was the type with youngest age, with a median age of 44 years ; while esophageal cancer was the type with oldest age, with a median age of 65 years. Besides breast cancer and uterine cancer, thyroid cancer was more common in women than in men, while other common malignant tumors were more common in men. Compared with the national tumor data in 2012, the constituent ratios of common malignant tumors in Chongqing data of SMIM for malignant tumor in 2015-2016 were obviously different（P 〈 0.05, except the uterine cancer constituent ratio in Chongqing in 2015）. Comparing the 2015 data with the 2016 data of Chongqing SMIM for malignant tumor,the differences of same tumor＇s constituent ratio were not significant （except esophageal cancer and thyroid cancer,P 〈 0.05）. Conclusion According to the gender and age characteristics of common malignant tumors reflected in SMIM, differential tumor screening should be performed at different ages for men and women in Chongqing.
