摘要
目的 探讨镰刀状肾实质切开对犬肾功能的影响。方法 将 1 8条成年犬分 3组 :实验组为镰刀状肾实质切开组 (A组 )、对照组为肾后唇中下 1 / 3肾实质切开组 (B组 )和无萎缩性肾实质切开组 (C组 )。观测 :术前 ,术后 1d、3d,1周、2周尿微量白蛋白 (ALB)、尿N 乙酰 β氨基葡萄糖酐酶 (NAG) ;术后 1周、1个月排泄性尿路造影 (IVU) ;术后 6月肾组织光镜和电镜结构。结果术后 1d、3d、1周、2周尿ALB和NAG值A组分别为 2 87.45± 8.2 3和 3 .45± 0 .2 8、32 7.87± 1 0 .98和 5 .2 5± 0 .48、468.36± 2 0 .66和 7.87± 0 .65、2 1 2 .87± 1 3 .65和 3 .0 1± 0 .1 7,与B、C两组比较 ,A组明显低于C组 (P <0 .0 1 ) ,A、B组间差异无显著性 (P >0 .0 5) ;术侧肾显影延缓时间A组 (1 35± 1 2 )s明显短于C组 (2 67± 2 8)s(P <0 .0 1 ) ,而与B组 (1 2 8± 1 5)差异无显著性 (P >0 .0 5) ;A、B组肾脏的病理改变明显轻于C组。结论 镰刀状肾实质切开对肾功能影响小 。
Objective To evaluate the influence of sickle shaped renal parenchyma incision on renal function of dogs.Methods Eighteen healthy adult dogs were divided into 3 groups: sickle shaped renal parenchyma incision group (group A) as experimental group, middle inferior 1/3 of posterior renal parenchyma incision group (group B) and anatrophic renal parenchyma incision group (group C) as control groups. The urine ALB and NAG were detected before operation and 1 day, 3 days, one week, two weeks after operation. Intravenous pyelography was performed 1 week and 1 month after operation. The renal histology character was observed 6 months after operation.Results The urine ALB and NAG 1 day, 3 days, one week, two weeks after operation in group A were 287.45±8.23 and 3.45±0.28, 327.87±10.98 and 5.25±0.48, 468.36±20.66 and 7.87±0.65, 212.87±13.65 and 3.01±0.17 respectively, all significantly lower than in group C (P<0.01), but having no significant difference with group B (P>0.05). The renal visualization delayed time in group A was shorter than in group C. The renal pathology change of groups A and B were milder than in group C.Conclusion Sickle shaped renal parenchyma incision has a little influence on renal function and is an ideal method to deal with complicated staghorn calculus.
