摘要
目的探讨幽门螺杆菌(Hp)感染与2型糖尿病并发症的关系。方法将116例2型糖尿病患者根据尿素酶试验和Giemsa染色检测结果分为Hp阳性组(54例)和Hp阴性组(62例)。2组糖尿病患者保持原治疗方法不变,分别检测2组血清总胆固醇(TC)、三酰甘油(TAG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、内皮素(ET)的水平,观察2组并发症(心血管病、脑血管病、糖尿病肾病、糖尿病眼底改变、糖尿病足)发生率。结果 Hp阳性组血清TC、TAG、LDL-C、ET水平均明显高于Hp阴性组,血清HDL-C水平和心血管病、脑血管病、糖尿病肾病、糖尿病眼底改变、糖尿病足发生率均明显低于Hp阴性组(均P<0.05)。结论 Hp感染可使2型糖尿病患者ET升高,血脂代谢异常,并增加糖尿病并发症发生率。
Objective To investigate the relationship between Helicobacter pylori(Hp)infection and complications of type 2 diabetes mellitus(DM).Methods According to urease test and Giemsa staining,116 cases of type 2 DM were divided into Hp-positive group(54 cases)and Hpnegative group(62 cases).The two groups of patients with diabetes unchanged of the keep original treatment method unchanged.Serum levels oftotal cholesterol(TC),triglyceride(TAG),low density lipoprotein cholesterol(LDL-C),high density lipoprotein cholesterol(HDL-C)and endothelin(ET)and incidences of cardiovascular disease,cerebrovascular disease,diabetic nephropathy,diabetic fundus lesion and diabetic foot ulceration were measured in both groups.Results Serum levels of TC,TAG,LDL-C and ET in Hp-positive group were significantly higher than those in Hp-negative group(P<0.05).Serumlevels of HDL-C and incidences of cardiovascular disease,cerebrovascular disease,diabetic nephropathy,diabetic fundus lesion and diabetic foot ulceration in Hp-positive group were obviously lower than those in Hp-negative group(P<0.05).Conclusion Hp infection can elevate the level of ET,cause the abnormity of blood lipid metabolismand increase the incidence of diabetic complicationsin patients with type 2 DM.
