前列腺移行区癌T_2WI影像特征的多因素分析 被引量:6

Multivariate Logistic Regression Analysis for Diagnosis of Transition Zone Prostate Cancer on T_2WI

摘要 目的探讨T_2WI影像特征对移行区前列腺癌(PCa)的诊断价值并筛选诊断价值较高的影像征象。方法回顾性分析经过病理证实的43例移行区PCa的T_2WI图像,另选取91例前列腺增生患者作为对照组,采用多因素Logistic回归筛选影像征象,将筛选出的影像征象联合,计算联合诊断的敏感度、特异度、阳性预测值、阴性预测值及准确率。结果 T_2WI上7种影像征象中,多因素Logistic回归筛选出3种影像征象:病灶形态、前列腺包膜、患侧移行区与外周带分界。联合诊断中,形态+前列腺包膜的特异度、阳性预测值、准确率最高,分别为88.5%、67.8%、83.41%,形态+患侧移行区与外周带分界的敏感度、阴性预测值最高,分别为82.8%、93%。结论 T_2WI上病灶形态、前列腺包膜、患侧移行区与外周带分界是诊断移行区PCa的独立危险因素。 Objective To evaluate the diagnostic capabilities of T2W imaging features in transition zone prostate cancer and select imaging signs which have higher diagnostic value. Methods Retrospectively analyzed the characteristics of T2WI in 43 cases of transition zone prostate cancer and 91cases of benign prostatic hyperplasia. Eight indexes were screened by multivariate logistic regression analysis and the sensitivity,specificity,positive predictive value,negative predictive value and accuracy of associated imaging features which were selected,were all calculated. Results By multivariate logistic analysis,the statistically significant differences were found in lesion shape,prostatic capsule,boundary of transition zone and peripheral zone on T2WI. The lesion shape and prostatic capsule had the highest specificity,positive predictive value and accuracy,being 88. 5%,67. 8%,83. 41%,respectively. Lesion shape and boundary of transition zone and peripheral zone had the highest sensitivity and negative predictive value,being 82. 8%,93%,respectively. Conclusion In T2WI,lesion shape,prostatic capsule and boundary of transition zone and peripheral zone were independent risk factors of transition zone prostate cancer.
