摘要
目的研究超声对胎儿心脏畸形筛查的临床诊断价值。方法纳入2015年1月至2017年6月期间在我院行超声检查并分娩的妊娠20~24周孕妇作为研究对象,观察不同切面超声对胎儿心脏畸形的诊断结果。结果不同单一切面诊断胎儿心脏畸形的检出率均较高,其中以四腔心切面最高,达到100%,其余切面的检出率均在90%以上,不同切面的检出率无统计学差异(P>0.05)。胎儿先天心脏畸形检出类型以室间隔缺损最多(31.3%),明显高于其他类型(P<0.05)。多切面联合检查的灵敏度高于四腔心切面联合流出道切面以及单一四腔心切面(P<0.05)。结论四腔心切面联合流出道切面对胎儿心脏畸形的检出率较高,可作为胎儿先天性心脏畸形筛查的主要手段。对于较难诊断或诊断不清的胎儿,超声多切面联合检查可提高胎儿心脏畸形的检出率。
Objective To study the clinical diagnostic value of ultrasound on fetal cardiac malformation. Methods Pregnant women with 20 to 24 weeks of pregnancy who received ultrasound examination and delivery in our hospital from January 2015 to June 2017 were selected as research objects. The diagnostic results to cardiac malformation of ultrasound in different aspect were observed. Results The single detection rates of each view were higher; Four-chamber view had the highest detection rate (100%), and the other views of aspects were all higher than 90%; No statistical difference was found in the detection rate among different views of aspects (P〉0.05). Ventricular septal defect was the most common type of cardiac malformation detected by ultrasound, accounted for 31.3%, significantly higher than other types (P〈0.05). The sensitivity of multi-view joint examination was significantly higher than that of four-chamber view and four-chamber view combined with outflow tract view (P〈0.05). Conclusions Four-chamber view combined with outflow tract view has relative high detection rate on fetal cardiac malformation, which can be used as the main method for screening of fetal cardiac malformation. For fetus with unclear diagnosis or difficulty in diagnosis, multi-view joint ultrasound examination can improve the detection rate of fetal cardiac malformation.
