ERCP诊治胰十二指肠切除术后胆肠吻合口远期并发症分析 被引量：5

摘要 目的分析并总结采用经内镜逆行性胰胆管造影术(ERCP)诊治的胰十二指肠切除术(Whipple手术)后胆肠吻合口远期并发症的诊治情况。方法对发生胆肠吻合口远期并发症的7例Whipple术后患者,由熟练的ERCP医师采用十二指肠镜通过胃肠腔进镜至空肠输入袢,找到胆肠吻合口后行插管逆行造影,再进行治疗,回顾性分析进镜成功率、治疗成功率和治愈率。结果本组7例患者共行ERCP 9例次:其中进镜未能成功1例;另有1例患者进镜较困难,内镜可观察到胆肠吻合口但内镜未能到达诊治位置,该例患者无法进行治疗;其余患者均行内镜下治疗。进镜成功率88.9%(8/9),治疗成功率77.8%(7/9),治愈率71.4%(5/7)。结论ERCP诊治Whipple术后胆肠吻合口远期并发症技术上可行,成功率高,效果好,可作为胰十二指肠切除术后胆肠吻合口远期并发症的首选诊疗方案。
