摘要
本研究旨在观察术前超声诊断甲状腺微小乳头状癌淋巴结转移的情况,探讨超声诊断在评估PTMC的临床应用价值。选取2012年1月至2015年1月经我院确诊并在我院接受手术治疗的PTMC患者75例作为研究对象,将同期来我院接受治疗的良性甲状腺病变患者63例作为对照对象。根据不同患者的具体情况调节对超声仪的频率、增益、深度范围及焦点,并在术后制成病理石蜡切片,比较诊断情况。研究表明,患者的超声诊断结果与病理切片结果与正常人群不同。在超声下可见包膜不完整、形态不规则、边界模糊不清、内部回声不均匀、肿块结节内微钙化情况的影像学特征;在病理切片镜下可见砂粒体、肿块边界浸润情况,囊性变或纤维假包膜的情况。以病理检查结果作为参照,超声诊断的符合率为78.79%,灵敏度为75.86%,特异度为100%。由此可推断,超声诊断可作为术前诊断癌细胞中央区淋巴结转移,评价术中处理措施的重要检查手段之一。
The purpose of this study was to observe the preoperative ultrasonography diagnosis of lymph node metastasis of papillary thyroid carcinoma, and explore the clinical application value of ultrasound in the assessment PTMC. Choose 75 cases of patients with PTMC through making a definite diagnosis and receiving surgical treat- ment in our hospital from January 2012 to January 2015 as the research object, choose the same time to the hospital for treatment of 63 patients with benign thyroid lesions as the control object. According to the specific conditions of different patients to adjust the depth of the frequency, gain, the scope and focus of the ultrasonic instrument, made the postoperative pathological paraffin section, and compare with the diagnosis. The research showed that the pa- tients' ultrasound diagnosis results and pathological results are different from the general population. Under the ul- trasound, we could see capsular is incomplete, shape is irregular, boundary is blurred, intemal echo is not uniform, and the imaging characteristics of microcalcification in tumor nodules; under the pathological section microscope, we can see the infiltration situation of sand body and the mass boundary, the situation of cystic change or fiber psue- docapsule. Take pathologic examination results as the reference, the coincidence rate of ultrasonic diagnosis was 78.79%, the rate of sensitivity was 75.86%, the rate of specificity was 100%. It can be concluded that ultrasonic diagnosis can be used as preoperative diagnose the central lymph node metastasis of cancer cell, and one of the important inspection means of the treatment measures in the postoperative evaluation.
