半髋关节置换和内固定治疗高龄股骨转子间骨折的近期疗效比较 被引量:9

摘要 目的比较半髋关节置换与股骨近端锁定钢板内固定两种方式治疗高龄股骨转子间骨折的近期临床疗效。方法将60例高龄Evens-Jensen分型Ⅱb型股骨转子间骨折患者按治疗方法分为两组,分别应用半髋关节置换(30例)与股骨近端锁定钢板(30例)治疗,比较两种手术的骨折愈合时间、髋关节功能Harris评分、并发症。结果患者均获得随访,时间3~12(8±2)个月。骨折愈合时间、并发症两组比较差异无统计学意义(P>0.05)。髋关节功能Harris评分优良率半髋关节置换组优于股骨近端锁定钢板组(P<0.05)。结论半髋关节置换可作为体质较差、预期寿命周期有限的高龄股骨转子间骨折患者的治疗方式。 Objective To compare the recent curative efficacy between hemiarthroplasty and proximal femoral locking plate internal fixation in the treatment of elderly patients with femoral intertrochanteric fracture.Methods The 60 elderly patients with Events-JensenⅡb intertrochanteric fracture were divided into two groups according to the treatment methods, hemiarthroplasty and proximal femoral locking plate were used respectively , each group of 30 cases, and then the fracture healing time , hip Harris score and postoperative complications of the two surgeries were compared.Results All cases were followed up for 3~12(8 ±2) months.There was no significant difference between the two groups in the fracture healing time and the postoperative complication (P〉0.05).The superior rate of hip Harris score in the hemiarthroplasty group excelled the proximal femoral locking plate group ( P〈0.05 ).Conclusions Hemiarthroplasty can be applied as the treatment of elderly patients with femoral intertrochanteric fracture who have poor physical condition and limited life expectancy.
