摘要
目的探讨缺血修饰清蛋白(IMA)、N末端脑钠肽前体(NT-proBNP)及超敏肌钙蛋白Ⅰ(Tn I-Ultra)对诊断急性冠状动脉综合征(ACS)的应用价值。方法选取本院ACS及稳定型心绞痛(SAP)患者各30例,分别于干预前及干预后检测2组外周血IMA、NT-proBNP及Tn I-Ultra表达水平,并进行单个核细胞体外分离培养,予瑞舒伐他汀干预,ELISA检测干预后血单核细胞IMA、NT-proBNP及Tn I-Ultra的表达情况;记录患者胸痛发作时间,与上述指标进行相关性分析。结果 ACS患者的IMA、NT-proBNP及Tn I-Ultra表达水平明显高于SAP患者,差异有统计学意义(P<0.05);给予瑞舒伐他汀治疗后,2组外周血IMA、NT-proBNP及Tn I-Ultra水平均明显下降,且ACS组下降的更为明显,差异有统计学意义(P<0.05);相关性分析显示胸痛发作时间与IMA、NT-proBNP及Tn I-Ultra呈正相关。结论 IMA、NT-proBNP及Tn I-Ultra与ACS发作具有显著相关性,对ACS的诊断具有重要指导意义。
Objective To investigate the clinical value of ischemic modified albumin( IMA),N-terminal pro-brain natriuretic peptide( NT-proBNP) and ultra-sensitive troponin I( TnⅠ-Ultra) in the diagnosis of acute coronary syndrome( ACS). Methods 60 patients with ACS or stable angina pectoris( SAP) were enrolled in this study,with 30 cases in each group. The levels of IMA,NT-proBNP and TnⅠ-Ultra in peripheral blood were measured before and after intervention,and mononuclear cells were isolated and cultured in vitro. The expressions of IMA,NT-proBNP and TnⅠ-Ultra were measured by ELISA,and the correlation between the onset of chest pain and the above indexes was recorded. Results The levels of IMA,NT-proBNP and TnⅠ-Ultra in ACS patients were significantly higher than those in SAP patients,with the differences statistically significant( P〈 0. 05).After treatment with rosuvastatin,the levels of IMA,NT-proBNP and TnⅠ-Ultra in peripheral blood of the two groups decreased significantly,and the levels of them in the ACS group decreased more significantly,with the differences statistically significant( P〈 0. 05). The correlation analysis showed that the onset time of chest pain was positively correlated with IMA,NT-proBNP and TnⅠ-Ultra. Conclusion IMA,NT-proBNP and TnⅠ-Ultra have significant correlation with the incidence of ACS,which is of great significance to the diagnosis of ACS.
