肠内营养支持在胃肠道肿瘤患者围术期中的应用效果 被引量:1

摘要 目的研究胃肠道肿瘤疾病患者在围术期接受肠内营养支持的临床效果。方法选取2014年10月~2016年10月我院收治的84例胃肠道肿瘤疾病患者作为研究对象,随机将其分为对照组和研究组,每组各42例。两组患者在入院后均接受手术治疗。对照组围术期实施肠外营养支持;研究组围术期实施肠内营养支持。观察比较术前1 d、术后7 d患者的免疫指标、炎性指标、住院治疗总时间、营养不良事件发生率及满意度。结果两组患者术前1 d血清白蛋白(Alb)、转铁蛋白(TRF)、总蛋白(TP)、前白蛋白水平(PAB)等指标比较,差异无统计学意义(P>0.05);术后7 d观察组TRF、PAB指标[(1.73±0.19)g/L、(173.52±22.36)mg/L]高于对照组[(1.46±0.20)g/L、(140.36±27.41)mg/L],差异有统计学意义(P<0.05)。研究组患者术后住院治疗总时间[(11.19±1.34)d]短于对照组[(15.27±2.06)d],差异有统计学意义(P<0.05);研究组营养不良事件的发生率(2.4%)低于对照组(17.4%),差异有统计学意义(P<0.05);研究组患者对术后营养支持方式的满意度(95.2%)高于对照组(78.6%),差异有统计学意义(P<0.05)。结论胃肠道肿瘤疾病患者在接受手术治疗期间实施肠内营养支持,可以保证术后机体营养供应,改善其免疫指标,加快恢复速度,降低发生营养不良事件的可能性。 Objective To study the clinical effect of perioperative enteral nutrition in patients with gastrointestinal can- cer.Methods Totally 84 patients with gastrointestinal cancer who were admitted to our hospital from October 2014 to October 2016 were randomly divided into control group and study group,with 42 cases in each group.Both groups re- ceived surgery after admission.The control group perioperative implementation of parenteral nutrition support,and the study group perioperative implementation of enteral nutrition support.Observe and compare the immune index,the in- flammatory index,the total time of hospitalization,the incidence of malnutrition and the satisfaction of patients on the first day before operation and the seventh day after operation.Results There was no significant difference in serum albu- min (ALB),transferrin (TRF),total protein (TP) and prealbumin (PAB) between the two groups before operation (P〉0.05). The levels of TRF and PAB ([1.73±0.19] g/L and [173.52±22.36] mg/L) were higher than those in the control group ([1.46±0.20] g/L and [140.36±27.41] mg/L),the difference was statistically significant (P〈0.05).The total postoperative hospitalization time in study group ([11.19~1.34] d) was shorter than that in control group ([15.27±2.06] d),the differ- ence was statistically significant (P〈0.OS).The rate of malnutrition occurred in study group (2.4%) was lower than that in control group (17.4%).The difference was statistically significant (P〈0.05).The satisfaction rate of postoperative nutri- tional support (95.2%) was higher than that in the control group (78.6%),the difference was statistically significant (P〈 O.05).Conclusion Gastrointestinal cancer patients undergoing enteral nutrition support during surgical treatment can ensure nutritional supply of postoperative body and improve their immune indexes,speed up the recovery and reduce the possibility of occurrence of malnutrition.
