Clinical Value of ^(18)F-FDG PET/CT Image Texture Features Analysis In The Diagnosis of Pulmonary Masses

摘要 目的探讨^(18)F-氟代脱氧葡萄糖(^(18)F-FDG)正电子发射断层显像术(PET)图像纹理特征参数(熵)对肺部肿块诊断的临床价值。方法搜集2014年1月至2015年12月在本院就诊的120例肺部肿块病变资料,进行回顾性研究,分析PET纹理特征参数(熵)对肺部肿块良恶性鉴别诊断的临床价值,并与最大标准摄取值(SUV_(max))诊断肺部肿块恶性病变的灵敏度、特异度和准确度相比较。结果^(18)F-FDG PET纹理特征参数熵诊断肺部肿块恶性病变的受试者工作特征(ROC)曲线下面积为0.713,有一定的诊断价值,当熵=5.6时,其诊断肺癌的灵敏度74.44%、特异度73.33%,准确度74.17%。SUV_(max)诊断肺部肿块恶性病变的ROC曲线下面积为0.802,有较高的诊断价值。当SUVmax=5.5时,灵敏度80.0%,特异度70.0%,准确性77.50%。纹理特征参数熵联合SUVmax诊断肺部肿块恶性病变的灵敏度为92.22%、特异性为60.0%,准确性84.17%。结论^(18)F-FDG PET图像纹理特征参数熵对肺部肿块良恶性鉴别诊断有一定价值,与SUVmax联合应用可提高肺部肿块临床诊断的灵敏度和准确度。 Objective To explore the clinical value of18F-FDG PET image texture feature parameters( entropy) in the diagnosis of pulmonary masses. Methods 120 patients with pulmonary masses in our hospital from January 2014 to December 2015 were collected. A retrospective study was carried out to analyse the clinical value of18F-FDG PET texture pa-rameters( entropy) in differential diagnosis of benign and malignant lung tumors,and comparison of sensitivity,specificity and accuracy with SUVmaxdiagnosis of malignant lesions in lung masses. Results The area under the ROC curve of18FFDG PET texture feature parameter entropy diagnosis of lung tumor malignant lesions is 0. 713. There is a certain diagnostic value. When the entropy = 5. 6,the sensitivity of the diagnosis of lung cancer = 74. 44%,specificity = 73. 33%,accuracy= 74. 17%. ROC area under the SUVmaxcurve was 0. 802,which had higher diagnostic value in the diagnosis of malignant lesions of lung masses. When SUVmax= 5. 5,the sensitivity was 80%,the specificity was 70%,the accuracy was 77. 50%.The sensitivity of texture feature parameter entropy combined with SUVmaxin the diagnosis of lung tumor malignant lesions was 92. 22%,the specificity was 60%,and the accuracy was 84. 17%. Conclusion18F-FDG PET image texture feature parameter entropy has a certain value in the differential diagnosis of benign and malignant lung tumors,and the combined application of SUVmaxcan improve the sensitivity and accuracy of the clinical diagnosis of pulmonary masses.
