摘要
目的:探讨免疫组化检测Napsin A对非小细胞肺癌诊断的应用价值。方法:计算机检索Pubmed、中国学术期刊全文数据库、维普中文科技期刊数据库、万方数据库、中国生物医学文献数据库,末次检索日期为2017年2月27日。按照纳入与排除标准筛选文献,使用QUADAS对文献进行质量评价,以诊断比值比(DOR)为效应评价指标,采用Meta Disc1.4和Rev Man5.0诊断试验Meta分析方法检验各文献间的异质性,并根据异质性结果选择相应的效应模型进行加权定量合并,计算敏感度和特异度及其95%CI。绘制受试者工作特征曲线(SROC)并计算曲线下面积(AUC)。结果:免疫组化检测Napsin A在非小细胞肺癌诊断的汇总敏感度和特异度分别为0.87(95%CI:0.84~0.89)和0.89(95%CI:0.86~0.91),DOR为761.24(95%CI:51.77~11194.01),SROC的AUC为98.95%,Q*指数为0.939 7。结论:免疫组化检测Napsin A诊断非小细胞肺癌,特异度高,灵敏度较高,具有一定的临床应用价值。
Objective: To discuss the value of immunohistochemical( IHC) staining of Napsin A in detecting lung cancer. Methods: The Pubmed,CNKI,CBM,VIP and Wang Fang databases were searched for such as to identify all the relevant case-control trials. The deadline of retrieval was Feb. 27,2017. Then further screening of the articles based on the inclusion and exclusion criteria was carried. The quality of the included studies were assessed by QUADAS. Meta Disc1. 4 和 Rev Man5. 0 were adopted to analyze the data by calculating the sensitivity and specificity and 95% CI. Results: A total of 7 case-control studies were included in the study. The pooled sensitivity was 0. 87( 95% CI: 0. 84-0. 89) and the pooled specificity was 0. 89( 95% CI:0. 86-0. 91). The DOR was 761. 24( 95% CI: 51. 77-11194. 01),the AUC of SROC were 98. 95%,Q~* values were 0. 939 7. Conclusion: The specificity and sensitivity of IHC method for Napsin A detection in lung cancer were relatively high. The IHC Method detecting Napsin A was useful in differential diagnosis for lung cancer.
