摘要
目的研究分析心型脂肪酸结合蛋白(h-FABP)在轮状病毒(Rotavirus)患儿中的变化及临床意义。方法选取我院2015年5月—2016年8月儿科门诊收治入院的轮状病毒感染患儿43例作为观察组,选取同期轮状病毒检测阴性的普通腹泻患儿60例作为对照组。在入院后不同时间段内,分别检测观察组和对照组心型脂肪酸结合蛋白(h-FABP)、肌钙蛋白I(c TNI)以及肌酸磷酸激酶同工酶(CK-MB)的浓度,并采用统计软件分析所得数据。结果在入院后的各个时间段里,观察组的血清HFABP、cTnI及CK-MB均明显高于对照组,差异有统计学意义(P<0.01);h-FABP升高时间比cTNI及CK-MB要早,在入院后2 h达到峰值17.12±5.63 ng/ml,24小时左右恢复到正常水平,血清cTNI、CK-MB水平则在入院后8 h左右达到峰值0.18±0.05μg/L、42.3±9.5 U/L;入院后2 h,h-FABP阳性率达到95.3%,明显高于cTnI及CK-MB,入院后24 h及48 h,h-FABP阳性率均明显低于cTnI及CK-MB。结论血清h-FABP可作为RV患儿早期心肌损伤的生化检验标志物,对轮状病毒感染的早期诊断、指导治疗具有重要的临床意义。
Objective To investigate the changes and clinical significance of heart fatty acid binding protein( h-FABP) in children with rotavirus. Methods We selected 43 cases of rotavirus infection in our hospital from May 2015 to August 2016 as the observation group,and another 60 cases of diarrhea with negative rotavirus in the same period as the control group. In different periods after admission,we detected the content of heart type fatty acid binding protein( h-FABP),troponin I( cTNI) and creatine kinase isoenzyme( CK-MB) in the observation group and the control group,and analyzed the data by Chi-square test. Results In each period after admission,the levels of serum h-FABP,cTNI and CK-MB in the observation group were significantly higher than those in the control group( P < 0. 01). The level of h-FABP increased earlier than that of cTNI and CK-MB,reaching the peak of 17. 12 ± 5. 63 ng/ml at 2 h after admission and resuming to normal level 24 hour after admission. The levels of serum cTNI and CK-MB reached the peak of 0. 18 ± 0. 05 μg/L,42. 3 ±9. 5 U/L at 8 h after admission. the positive rate of h-FABP was 95. 3% 2 hour after admission,significantly higher than that of cTnI and CK-MB. The positive rate of h-FABP was significantly lower than that of cTnI and CK-MB at 24 hour and 48 hour after admission. Conclusion Serum h-FABP can be a biochemical marker for early myocardial damage in children with RV,and has important clinical significance for early diagnosis and guiding treatment of rotavirus infection.
