低辐射剂量迭代技术在颅脑螺旋CT三维成像中的临床应用 被引量:1

摘要 目的探讨低辐射剂量迭代技术在颅脑螺旋CT三维成像中的临床应用价值。方法收集郴州市第一人民医院2016年7月至2017年6月接受颅脑CT扫描成像的可疑病变患者230例的临床资料,分为常规剂量非螺旋扫描组(Ⅰ组)和低辐射剂量迭代技术螺旋扫描组(Ⅱ组),每组115例,原始图像和重建图像均传送至GEAW4.2工作站,分别由3名CT医生进行后处理,采用4分制独立对两组影像质量进行评价,采用t检验对比两组辐射剂量、影像质量的客观指标(噪声)及主观评分。结果与Ⅰ组比较,Ⅱ组显著降低了辐射剂量,差异有统计学意义(P<0.01),Ⅱ组的平均有效剂量为(0.72±0.16)m Sv,降低率达63.45%,单次扫描容积CT剂量指数(CTDIvol)、总剂量-长度乘积(DLP)的降低率分别为84.74%和63.44%。Ⅱ组与Ⅰ组的影像质量比较,差异无统计学意义(P>0.05)。结论应用低辐射剂量迭代技术进行颅脑螺旋CT三维成像,不仅保证了影像质量,而且显著降低了辐射剂量,具有较好的临床应用价值。 Objective To explore the clinical application value of low radiation dose iterative technique in craniocerebral spiral CT three-dimensional imaging. Methods Two hundreds and thirty patients with suspected lesions receiving craniocerebral scanning imaging in the Chenzhou Municipal First People′s Hospital from July 2016 to June 2017 were collected and divided into the routine dose group(Ⅰ)and low radiation dose iterative technique spiral scanning group(Ⅱ),115 cases in each group. The original image and reconstructed image were transmitted to the GEAW4.2 workstation,and were performed the aftertreatment by three CT doctors. The 4-points scale was adopted to independently evaluate the image quality in the two groups. The t test was used to compare the radiation dose,objective indicators(noise)and subjective scores of image quality between the two groups. Results Compared with the groupⅠ,the radiation dose in the group Ⅱ was significantly decreased,the difference was statistically significant(P〈0.01).The average effective dose in the group Ⅱ was(0.72±0.16)m Sv,the reduction rate reached 63.45%. The reduction rates of CTDIvoland DLP were 84.74% and 63.44% respectively. The image quality had no statistical difference between the groupⅠ and group Ⅱ(P〈0.05). Conclusion Using low radiation dose iterative technique for conducting craniocerebral spiral CT three-dimensional imaging not only ensures the image quality,but also significantly reduces the radiation dose,and has good clinical application value.
