摘要
目的探讨超声监测子宫瘢痕缺陷对再妊娠风险评估的价值。方法选取138例子宫瘢痕再妊娠患者,根据彩色多普勒超声检查结果对瘢痕进行分级(Ⅰ级组82例,Ⅱ级组30例,Ⅲ级组21例,Ⅳ级组5例),比较各级患者子宫前壁下段厚度、分娩出血量、分娩中缩宫素用量、分娩方式,以及子宫破裂发生率、严重感染发生率、分娩后子宫出血发生率等情况。结果 138例患者中,子宫瘢痕缺陷发生率为40.58%(56/138);子宫前壁下段厚度由大到小依次为Ⅰ级组、Ⅱ级组、Ⅲ级组及Ⅳ级组,组间比较差异均有统计学意义(均P<0.05);分娩出血量、缩宫素用量、子宫破裂发生率、严重感染发生率及分娩后子宫出血发生率由低到高依次为Ⅰ级组、Ⅱ级组、Ⅲ级组及Ⅳ级组,组间比较差异均有统计学意义(均P<0.05);阴道分娩率由高到低依次为Ⅰ级组、Ⅱ级组、Ⅲ级组及Ⅳ级组,组间比较差异均有统计学意义(均P<0.05)。结论超声监测子宫瘢痕缺陷有助于瘢痕子宫再妊娠风险的评估,为科学选择分娩方式提供依据。
Objective To investigate the value of ultrasound monitoring the defects of uterine scar in evaluating the risk of re -pregnancy. Methods One hundred of thirty-eight patients with uterine scar re-pregnancy were enrolled.All patients wereexamined by color Doppler ultrasonography, the uterine scar was graded according to the ultrasonographic findings (8 2 cases of grade I group, 30 cases of grade n group, 21 cases of grade m group and 5 cases of grade w group).The thickness of the anterior wall of the uterus, blood loss during delivery, oxytocin dosage during delivery, delivery way, incidence of uterine rupture, serious infection rates and incidence of uterine bleeding after delivery were compared.Results In 138 patients, the incidence of uterine scar defect rate was 40.58%(56/138 ).The thickness of the anterior wall of the uterus of grade I group 〉grade n group 〉 grade m group 〉grade W group, the difference were statistically significant (all P〈0.05).The blood loss during delivery, oxytocin dosage during delivery, incidence of uterine rupture, serious infection rates and incidence of uterine bleeding after delivery of grade I group 〈grade n group 〈grade m group 〈grade W group, the difference were statistically significant (all P〈0.05).The vaginal delivery rate of grade I group 〉grade n group 〉grade m group 〉grade W group, the difference were statistically significant (all P 〈0.05).Conclusion Ultrasound monitoring of uterine scar defects can helpful to assessing the risks of scar uterus re -pregnancy, which provides the basis for scientificly choosing childbirth way.
