高龄男性血清Ⅰ型原胶原氨基端前肽水平变化的独立相关因素研究

摘要 目的探讨高龄(≥80岁)男性血清I型原胶原氨基端前肽(P1NP)水平变化的独立相关因素。方法对495例高龄男性进行伴随疾病和生活方式的调查,并进行血清P1NP、血生化、性激素和贫血等指标的检测,运用多元回归分析方法筛选影响高龄男性血清P1NP水平变化的独立相关因素。结果经单因素分析,2型糖尿病、晒太阳、室外运动、喝牛奶、甲状旁腺素(PTH)、25-羟基维生素D、白蛋白、肌酐、血钙、血磷、血铁、叶酸、铁蛋白与高龄男性血清P1NP水平有关(均P<0.05)。经多元回归分析,PTH、室外运动、血钙、血磷、血肌酐均是影响高龄男性血清P1NP水平变化的独立相关因素(均P<0.05)。结论积极防治肾功能损害、降低血清PTH水平、养成良好的生活方式,可能是降低高龄男性血清P1NP水平的重要方法。 Objective To probe into independent factors associated with serum N-terminal propeptide of type 1 procollagen(P1 NP) in men of advanced age. Methods A cross-sectional survey was conducted in a cohort comprising 495 men aged ≥80 years. All subjects underwent a survey on underlying chronic diseases and lifestyle. The serum levels of bone metabolism markers(P1 NP), parathyroid hormone(PTH), 25-hydroxyvitamin D(25-OH-VD), biomedical index, anemia-related index as well as sex steroids were detected. Univariate analysis and multivariate regression analysis were performed to analyze the independent factors associated with P1 NP. Results The univariate analyses demonstrated that P1 NP was significantly related to type 2 diabetes mellitus, sun exposure, exercise, milk consumption, PTH, 25-OH-VD, albumin, creatinine, calcium,phosphate, iron, folic acid, ferritin levels(all P <0.05). The multivariate regression analysis revealed that exercise, and PTH,calcium, phosphate, creatinine levels were independent correlative factors with P1 NP levels in men aged 80 and over(all P<0.05). Conclusion To decrease the serum concentrations of parathyroid hormone and creatinine, and healthy lifestyle may be beneficial for serum P1 NP level in men of advanced age.
