CysC、β2-MG、RBP、HbA1c和hs-CRP联合诊断糖尿病早期肾损伤的临床意义 被引量:10

摘要 目的探讨血清胱抑素C(CysC)、β2-微球蛋白(β2-MG)、视黄醇结合蛋白(RBP)、糖化血红蛋白(Hb A1c)、超敏C-反应蛋白(hs-CRP)联合诊断糖尿病早期肾损伤的临床意义。方法选取2015年9月至2017年6月期间我院100例糖尿病早期肾损伤患者设为早期肾损伤组,100例单纯糖尿病患者设为糖尿病组,100例健康人员设为健康组。检测并比较三组的CysC、β2-MG、RBP、Hb A1c和hs-CRP水平。结果三组的CysC、β2-MG、RBP、Hb A1c和hs-CRP水平依次为健康组<糖尿病组<早期肾损伤组(P<0.05);早期肾损伤组患者的CysC、β2-MG、RBP阳性率分别为67.00%、60.00%、71.00%;联合检测诊断糖尿病早期肾损伤的敏感性为91.00%,特异性为97.00%,阳性预测值为93.81%,阴性预测值为95.57%,有效性为95.00%。结论 CysC、β2-MG、RBP、Hb A1c和hs-CRP是糖尿病早期肾损伤的敏感因子,联合检测诊断糖尿病早期肾损伤效果显著。 Objective To investigate the clinical significance of CysC, β2-MG, RBP, HbAlc and hs-CRP in the diagnosis of early diabetic renal injury. Methods From September 2015 to June 2017 in our hospital, 100 patients with early diabetic renal injury, 100 patients with diabetes, and 100 healthy subjects were set as the early renal injury group, diabetic group and healthy group. The levels of CysC, β2-MG, RBP, HbAlc and hs-CRP of three groups were detected and compared. Results The levels of CysC, β2-MG, RBP, HbAlc and hs-CRP in early renal injury group, diabetic group and healthy group decreased successively (P〈0.05). The positive rates of CysC,β2-MG and RBP of early renal injury group were 67.00%, 60.00% and 71.00% respectively. The sensitivity, specificity, positive predictive value, negative predictive value and effectiveness of combined detection in the diagnosis of early diabetic renal injury were 91.00%, 97.00%, 93.81%, 95.57% and 95.00% respectively. Conclusions CysC, β2-MG, RBP, HbA 1 c and hs-CRP are the sensitive factors of early diabetic renal injury, and the combined detection has significant effect in the diagnosis of diabetic renal injury.
