摘要
目的探讨妊娠期甲状腺功能亢进患者血清甲状腺功能和免疫含量变化及其在临床中的应用效果。方法选取2015年5月~2016年12月我院收治的122例妊娠期甲状腺功能亢进患者作为观察组,另选取该时间段的120例妊娠期非甲状腺功能亢进孕妇作为对照组,比较两组在妊娠不同时期的免疫功能及甲状腺功能变化。结果观察组妊娠早、中、晚期时的TSH水平低于对照组,观察组妊娠早期的T3、FT3、T4、FT4水平均高于对照组,差异有统计学意义(P<0.05)。观察组妊娠期早、中期时的TRAb、Anti-TG及TPOAb阳性率显著高于对照组,差异有统计学意义(P<0.05)。两组妊娠晚期时的TRAb阳性率比较,差异无统计学意义(P>0.05)。观察组妊娠晚期时的Anti-TG及TPOAb阳性率高于对照组,差异有统计学意义(P<0.05)。结论妊娠期甲状腺功能亢进患者血清甲状腺功能和免疫含量不同时期均存在明显变动,针对此类患者临床需予以高度重视,及早诊断与治疗,定期检测甲状腺功能,改善母婴结局。
Objective To explore the changes of serum thyroid function and immune content in patients with hyperthyroidism during pregnancy and its clinical application.Methods 122 pregnant patients with hyperthyroidism treated from May 2015 to December 2016 in our hospital were selected as the observation group,120 pregnant women without hyperthyroidism were selected as the control group at the same time.The immune function and thyroid function at the different gestational periods between the two groups were compared.Results The index of TSH at the early,mid,late trimester of pregnancy in the observation group was lower than that in the control group,the level of T3,FT3,T4,FT4 at the early,mid,late trimester of pregnancy in the observation group was higher than that in the control group,with significant difference (P〈0.05).The positive rate of TRAb,Anti-TG and TPOAb at the early,mid trimester of pregnancy in the observation group was higher than that in the control group,with significant difference (P〈0.05).There was no significant difference in the positive rate of TRAb between the two groups (P〉0.05).The positive rate of Anti-TG and TPOAb at the late trimester of pregnancy in the observation group was higher than that in the control group,with significant difference (P〈0.05).Conclusion The thyroid function and immunoglobulin contents at the different gestational periods are significantly different.It is necessary to attach great importance to this kind of patient,to diagnose and treat early,to check the thyroid function regularly and improve the outcome of maternal and infant.
